右上肢严重毁损伤显微外科治疗一例

摘　　要：本文报道2016年10月西安兵器工业五二一医院手外科收治的1例右上肢严重毁损伤患者,亚急诊采用游离分叶ALTPF修复右前臂及右手尺侧创面,游离右足[⻊母]甲瓣再造右拇指,游离左足第2趾再造右示指,术后第2天出现皮瓣静脉危象,急诊行探查治疗最终取得成功。术后3年随访,患肢外观恢复良好,患肢肘、腕关节屈伸活动正常,再造拇、示指对捏功能恢复良好。This is a report of a complex defects in the right upper extremity treated in the Department of Hand and Microsurgery,521 Hospital of Norinco Group,in October 2016.The patient received multi-transfers of flaps for one-stage reconstruction,including a double-paddled ALTPF and a compound flap of the great toe and the second toe.Vascular compromises occurred in flaps on the 2nd day after surgery.All the flaps were rescued and survived completely after surgical exploration.Followed-up at 3 year after surgery showed that the appearance and function of the affected limb recovered well.The flexion and extension of elbow and wrist were normal.The pinch function of the reconstructed thumb and index finger recovered well.
