利培酮临床药物不良反应103例及合理用药分析

摘　　要：目的分析利培酮药物不良反应的发生特点和分布规律,为临床合理用药提供参考依据。方法回顾性收集广东省肇庆市第三人民医院2006年12月—2020年12月上报至国家药物不良反应监测中心的利培酮药物不良反应报告103例,分别对性别、年龄、给药剂量、给药频次、累及系统/器官、预后、对原疾病的影响等进行统计分析。结果103例不良反应报告中,60岁以上的老年患者居多,男女比例相当;当剂量<4 mg时,每天1次给药方式的不良反应发生率比其他剂量组高;累及的器官/系统以神经系统(46.56%)和皮肤及其附件(21.37%)为主,常见临床表现为锥体外系反应、皮疹、瘙痒和嗜睡等。大多数不良反应经减药、停药或对症处理后,临床症状均能得到良好控制。结论临床应加强对利培酮不良反应的监测,如出现不良反应,应根据患者的实际情况,采取对症处理等措施,即使日剂量<4 mg,仍建议遵说明书要求,按每天2次给药。Objective To analyze the characteristics and distribution of adverse drug reactions of risperidone,and to provide reference for clinical rational drug use.Methods A total of 103 adverse drug reactions reports of risperidone from the Third People′s Hospital of Zhaoqing City,Guangdong Province from December 2006 to December 2020 were retrospectively collected and reported to the National Adverse Drug Reaction Monitoring Center.The gender,age,dose,frequency of administration,system/organ involvement,prognosis and effect on the original disease were statistically analyzed.Results Among the 103 adverse drug reactions reports,the majority of elderly patients were over 60 years old,and the proportion of men and women was equal.When the dose was less than 4 mg,the incidence of adverse reactions of once-daily administration was higher than that of other dose groups.The main organs/systems involved were nervous system(46.56%)and skin and its accessories(21.37%).The common clinical manifestations were extrapyramidal reaction,rash,pruritus and lethargy.The clinical symptoms of most adverse reactions can be well controlled after drug reduction,drug withdrawal or symptomatic treatment.Conclusion Clinical monitoring of adverse reactions of risperidone should be strengthened.If adverse reactions occur,symptomatic treatment should be taken according to the actual situation of patients.Even if the daily dose is less than 4 mg,it is recommended to follow the instructions and administer risperidone twice a day.
