通腑醒神汤治疗高血压脑出血微创术后患者神经损伤的临床疗效及对脑水肿体积、血清因子的影响分析

摘　　要：目的:探析通腑醒神汤对高血压脑出血患者行微创术后神经损伤的治疗效果及对患者脑水肿体积和血清因子的影响。方法:选取2020年6月-2021年6月至薛城区人民医院接受治疗的高血压脑出血患者80例作为研究对象,所有患者均统一接受微创术治疗且术后均存在不同程度的神经受损,按计算机分组法将所有患者随机分为研究组和对照组,每组40例,对照组术后实施常规治疗,研究组在此基础上采用通腑醒神汤配合治疗,对比两组患者的治疗有效率,并对治疗前后的神经功能恢复情况、血清基质金属蛋白酶9(Matrix Metalloproteinase 9,MMP9)、白细胞介素-6(Interleukin-6,IL-6)等炎性指标及脑水肿体积变化情况进行对比。结果:研究组治疗总有效率高于对照组,其治疗后的美国国立卫生研究院卒中量表(National Institutes of Health Stroke Scale,NIHSS)评分、MMP-9、IL-6等血清炎性指标及脑水肿体积均低于对照组,Barthel指数高于对照组,以上数据经对比差异均具有统计学意义(P<0.05)。结论:通腑醒神汤治疗高血压脑出血患者微创术后神经功能受损的效果显著,可有效改善患者的神经功能,在有效控制其血清炎症因子水平的同时也可显著缓解其脑水肿症状,对提高治疗效果并改善患者的预后均有重要意义。Objective:To explore the therapeutic effect of the Tongfu Xingshen decoction(通腑醒神汤)on nerve injury after minimally invasive surgery in patients with hypertensive cerebral hemorrhage and its effects on cerebral edema volume and serum factors.Methods:A total of 80 patients with hypertensive intracerebral hemorrhage treated in Xuecheng District People’s Hospital from June 2020 to June 2021 were selected as the research subjects.All patients were uniformly treated by micro-wound surgery and had different degrees of nerve damage after surgery.According to the computer grouping method,all patients were randomly divided into the study group and the control group,with 40 cases in each group.The control group received routine treatment after operation,and the study group received the Tongfu Xingshen decoction more.The effective rate of treatment was compared between the two groups,and the recovery of neurological function,inflammatory indicators such as serum matrix metalloproteinase(MMP-9)and interleukin-6(IL-6),and the volume change of brain edema were compared before and after treatment.Results:The total effective rate in the study group was higher than the control group.After treatment,the NIHSS score,MMP-9,IL-6 and other serum inflammatory indexes and brain edema volume in the study group were lower than the control group,and the Barthel index was higher than the control group.The above data was statistically significant(P<0.05).Conclusion:The Tongfu Xingshen decoction has significant effect in the treatment of neurological impairment in patients with hypertensive cerebral hemorrhage after minimally invasive surgery,it can effectively improve the neurological function of patients,effectively control the level of serum inflammatory factors,and also significantly alleviate the symptoms of cerebral edema,which is of great significance to improve the treatment effect and ameliorate the prognosis of patients.
