温经通脉壮骨汤联合提拉推按复位法对神经根型颈椎病的疗效分析

摘　　要：目的:探析神经根型颈椎病患者采用温经通脉壮骨汤联合提拉推按复位法治疗的效果。方法:样本选取年限为2019年10月-2021年10月,样本为120例神经根型颈椎病患者,以数字表法分为对照组(60例)与观察组(60例),对照组采用提拉推按复位法治疗,观察组采用温经通脉壮骨汤联合提拉推按复位法治疗,比较两组临床疗效、治疗前后颈椎功能情况[颈部功能障碍指数(Neck Disability Index,NDI)]、疼痛程度[视觉模拟评分法(Visual Analogue Scale,VAS)]、血清疼痛因子水平[前列腺素E2(Prostaglandin E2,PGE2)、P物质(Substance P,SP)、基质金属蛋白酶3(Matrix Metalloproteinases 3,MMP3)],以及生活质量水平[日常生活能力评定量表(Activity of Daily Living Scale,ADL)]。结果:与对照组比较,观察组治疗总有效率明显更高(P<0.05);治疗前,两组患者NDI评分、VAS评分、ADL评分差异不大(P>0.05),治疗2周后,与对照组患者比较,观察组患者NDI评分、VAS评分更低,ADL评分更高(P<0.05);治疗前,两组患者PGE2、SP、MMP3等指标水平差异不大(P>0.05),治疗2周后,与对照组患者比较,观察组患者PGE2、SP、MMP3等指标水平更低(P<0.05)。结论:对神经根颈椎病患者采用温经通脉壮骨汤联合提拉推按复位法治疗可获得较好的效果,能够有效改善患者的颈椎功能,缓解疼痛感,提高生活质量,具有较高的临床推广价值。Objective:To explore the effect of the Wenjing Tongmai Zhuanggu decoction(温经通脉壮骨汤)plus lift-pull-push and press reduction in the treatment of cervical spondylotic radiculopathy.Methods:The samples were selected from October 2019 to October 2021,and 120 patients with cervical spondylotic radiculopathy were divided into the control group(60 cases)and the observation group(60 cases)according to the number table method.The control group was treated by the lift-pull-push and press reduction method,while the observation group was treated with the Wenjing Tongmai Zhuanggu decoction plus lift-pull-push and press reduction method.The clinical curative effect,cervical/neck functional disturbance(NDI)before and after the treatment,pain degree(VAS),serum levels of pain factor(PGE2,SP,MMP3)and life quality level of daily life(ADL)in two groups were compared.Results:Compared with the control group,the total effective rate in the observation group was significantly higher(P<0.05).Before treatment,there were no significant difference in NDI,VAS and ADL scores between the two groups(P>0.05).After 2 weeks of treatment,compared with the control group,the NDI and VAS scores in the observation group were lower,and the ADL score was higher(P<0.05).Before treatment,there was no significant difference in the levels of PGE2,SP,MMP3 and other indicators between the two groups(P>0.05).After 2 weeks of treatment,compared with the control group,the levels of PGE2,SP,MMP3 and other indicators in the observation group were lower(P<0.05).Conclusion:The Wenjing Tongmai Zhuanggu decoction plus lift-pull-push and press reduction can achieve good results in the treatment of patients with cervical spondylotic radiculopathy,it can effectively improve cervical spine function,relieve pain and improve quality of life,and has high clinical promotion value.
