体外发散式冲击波联合三七散治疗桡骨茎突狭窄性腱鞘炎的临床观察

摘　　要：目的:探讨体外发散式冲击波联合深圳平乐骨伤科医院三七散治疗桡骨茎突狭窄性腱鞘炎的临床效果。方法:选取深圳平乐骨伤科医院2020年2-2020年8月80例桡骨茎突狭窄性腱鞘炎患者,随机将患者分为联合组和对照组各40例,对照组仅采用MP-100体外发散式冲击波治疗仪治疗,联合组患者采取MP-100体外发散式冲击波治疗仪治疗的基础上再加院内制剂三七散外敷,对两组患者的临床治疗效果进行对比,根据VAS评分标准对患者的疼痛程度做记录和比较,以Cooney腕关节评分量表为标准对两组的腕关节功能作比较。结果:联合组治疗后的总有效率高于对照组,差异具有统计学意义(P=0.044 352)。联合组治疗后1周和治疗后3周的疼痛视觉模拟评分法(Visual Analogue Scale,VAS)评分均低于对照组,差异有统计学意义(P<0.05)。从Cooney腕关节评分量表的结果中得出对照组差级的例数高于联合组,组间差异具有统计学意义(P<0.05)。结论:采用体外发散式冲击波联合三七散外敷治疗桡骨茎突狭窄性腱鞘炎,临床治疗效果明显,可减少复发,提高医从性,且操作简单,具有良好的安全性和有效性。Objective:To investigate the clinical effect of extracorporeal divergent shock wave plus Sanqi San(三七散)in the treatment of radial styloid stenosing tenosynovitis.Methods:A total of 80 patients with radial styloid stenosynovitis in Shenzhen Pingle Orthopedics and Traumatology Hospital from February 2020 to August 2020 were selected and randomly divided into the combination group and the control group,with 40 cases in each group.The control group was only treated by MP-100 extracorporeal radiative shock wave therapy.The patients in the combination group were treated by MP-100 extracorporeal divergent shock wave therapy plus Sanqi San.The clinical treatment effect in the two groups was compared.The pain degree of the patients was recorded and compared according to the VAS score.The wrist function in the two groups was compared according to the Cooney wrist score scale.Results:The therapeutic effect in the combination group was always higher than the control group,and the difference was statistically significant(P=0.044352).The VAS scores in the combination group was lower than the control group at 1 week and 3 weeks after treatment,and the difference was statistically significant(P<0.05).According to the results of Cooney wrist scoring scale,the number of cases of difference grade in the control group was higher than that in the combination group,and the difference between groups was statistically significant(P<0.05).Conclusion:The clinical treatment effect of extracorporeal divergent shock wave therapy plus external application of Sanqi San is obvious in the treatment of radial stynovitis stenosis,it can reduce the recurrence,improve the medical flexibility of patients,and the operation is simple,with good safety and effectiveness.
