中药补肾活血汤结合中药直肠滴入治疗慢性肾病临床研究

摘　　要：目的:探究中药补肾活血汤结合中药直肠滴入治疗对慢性肾病患者中医证候积分及肾功能指标的影响。方法:前瞻性研究选取淄博市沂源县中医医院于2019年9月-2020年12月收治的126例慢性肾病患者为研究对象,采用随机数字表法分为对照组(63例)和观察组(63例)。对照组采取单纯中药补肾活血汤进行治疗,观察组予中药补肾活血汤结合中药直肠滴入治疗,两组均连续治疗2个疗程(15 d为一个疗程)。观察并对比两组患者治疗前后的肾功能指标[包括血肌酐(Serum Creatinine,Scr)、尿素氮(Blood Urea Nitrogen,BUN)、肌酐清除率(Creatinine Clearance,Ccr)]及中医证候积分(包括主症、次症单一及总积分)。结果:治疗前两组肾功能指标(Scr、BUN、Ccr)的差异无统计学意义(P>0.05),治疗后观察组肾功能指标(Scr、BUN、Ccr)改善幅度显著优于对照组(P<0.05);治疗后观察组各项主症单一(乏力倦怠、小便异常)及总积分、次症单一(面色无华、肢体不适、舌淡胖苔薄白、脉细带滑)及总积分较对照组显著下降(P<0.05)。结论:中药补肾活血汤结合中药直肠滴入治疗应用于慢性肾病患者中疗效显著,可有效改善患者的肾功能指标,降低中医证候积分,值得临床推广。Objective:To investigate the effect of the Bushen Huoxue decoction(补肾活血汤)plus rectal dripping of TCM medicine on TCM symptom score and renal function index of patients with chronic kidney disease.Methods:The prospective study selected 126 patients with chronic kidney disease who were admitted to Yiyuan County Hospital of Traditional Chinese Medicine in Zibo from September 2019 to December 2020 as the research subjects,and randomly divided them into the control group(with 63 cases)and the observation group(with 63 cases).The control group was treated with the Bushen Huoxue decoction,while the observation group was treated with the Bushen Huoxue decoction plus the rectal dripping of TCM medicine.Both groups were treated for two consecutive courses(15 days as one course).The renal function indexes[including blood creatinine(Scr),urea nitrogen(BUN),creatinine clearance rate(Ccr)]and TCM syndrome score(including main syndrome,secondary syndrome and total score)in the two groups of patients before and after treatment were observed and compared.Results:There was no difference in renal function indexes(Scr,BUN,Ccr)between the two groups before treatment(P>0.05).After treatment,the amelioration of renal function indexes(Scr,BUN,Ccr)in the observation group(combined strategy)was significantly better than that in the control group(single strategy)(P<0.05);after treatment,the main syndromes(fatigue and tiredness,abnormal urination)and total scores,and secondary syndromes(colorless complexion,limb discomfort,thin white tongue,thin and slippery pulse)and total scores in the observation group were significantly lower than those in the control group(P<0.05).Conclusion:The therapeutic effect of the Bushen Huoxue decoction plus rectal dripping of TCM medicine is significant in treating patients with chronic kidney disease,which can effectively improve the renal function indicators of patients,reduce the TCM syndrome scores,and is worthy of clinical promotion.
