针刺“外关穴、手三里穴”为主治疗膝骨性关节炎22例

摘　　要：膝骨性关节炎是一种以退行性改变为基础的疾病,其症状以疼痛、关节弹响或有摩擦音、屈伸受限甚至关节变形等为主,一般膝关节X线即可诊断,必要时可结合MRI或CT排除其他疾病。王建强副主任医师师承国医大师吕景山教授,擅长运用“施今墨对药”“吕景山对穴”治疗腰椎间盘突出症、颈椎病、肩周炎、膝痹、足跟痛等疼痛性疾病,受其“对法”学术思想的影响,王建强副主任医师在临床治疗疾病时取穴常以“对”为术,其运用“外关穴、手三里穴”治疗膝骨性关节炎就是其在临床中得到的经验。王建强副主任医师认为,膝痹多伴有外邪侵袭,取外关穴,遵《医宗金鉴》之言,其有祛邪外出之功,同时结合张介宾(号景岳)的四关学说,其有通达肘、膝关节之效;取手三里穴有同气相求以厚生化之源,关节对应取穴以舒筋止痛,其疗效可观,操作安全,可重复性高。火龙药灸是中医特色灸法之一,有温补阳气、通经活络的功效,对改善膝关节疼痛及活动度有积极作用。本研究采用的火龙药液是吕景山教授经验方经过特殊的制备方法加工提炼而成的,其操作规范,不良反应小,疗效可靠。王建强副主任医师采用针刺“外关穴、手三里穴”为主治疗膝骨性关节炎,并附验案说明,临床疗效显著,值得推广。Knee osteoarthritis is a disease based on degenerative changes,its symptoms are mainly pain,joint bounce or friction sound,limited flexion and extension and even joint deformation.Generally,X-ray can diagnose the knee joint arthralgia,and the Magnetic Resonance Imaging or the Computed Tomography can be assisted to exclude other diseases when it is necessary.The teacher of Deputy Chief Physician WANG Jianqiang is Professor LV Jingshan,a master of TCM.WANG Jiangiang is good at using“coupled medicinals”invented by SHI Jinmo and“coupled acupuncture points”invented by LV Jingshan to treat painful diseases such as lumbar disc herniation,cervical spondylosis,periarthritis of shoulder,arthralgia of knee and heel pain.Influenced by Professor LV Jingshan’s academic thought of“the method of connecting two drugs or acupoints”,WANG Jianqiang,often uses Professor LV Jingshan’s way as the technique in treating diseases in clinical practice.His clinical experience is to use Waiguan acupoint(SJ5)and Shousanli acupoint(LI10)to treat knee osteoarthritis.Deputy Chief Physician WANG Jianqiang believes that the knee arthralgia is accompanied with the invasion of external evil,therefore,taking Waiguan acupoint complies with Yizong Jinjian(《医宗金鉴》),it can remove the invasion of foreign evils,at the same time,combined with TCM master ZHANG Jiebin’s four passes theory,it has the effect of connecting elbow and knee joint.Taking Shousanli acupoint means that the joint corresponds to the point,the combination of the two acupoints means both exterior and interio complement each other,and the function of relaxing muscles and activating collaterals is better.The effect is considerable,the operation is safe,and the repeatability is high.Fire dragon medicine moxibustion is one of the characteristic moxibustion methods of traditional Chinese medicine.It has the effect of warming Yangqi(阳气),regulating meridian and activating collaterals,and has a positive effect on improving knee pain and range of motion.The liquid
