摘  要:目的探讨血清闭锁小带蛋白-1(ZO-1)、CC趋化因子配体2(CCL2)对胎膜早破合并宫内感染的诊断价值。方法选择2020年9月至2022年3月本院收治的141例胎膜早破产妇作为观察对象,根据胎儿是否足月,分为早产胎膜早破组73例和足月胎膜早破组68例,根据宫内是否感染将患者分为感染组和未感染组,另选择同期足月正常妊娠且为单胎的产妇70例为对照组。采用酶联免疫吸附(ELISA)法检测血清ZO-1、CCL2表达水平,ROC曲线分析血清ZO-1、CCL2对胎膜早破合并宫内感染的诊断价值。结果早产胎膜早破组、足月胎膜早破组、对照组孕周比较差异有统计学意义(P<0.05),各组产妇年龄、孕前BMI、初产妇比例比较,差异无统计学意义(P>0.05);与对照组比较,早产胎膜早破组、足月胎膜早破组产妇血清ZO-1水平降低,CCL2水平显著升高(P<0.05),早产胎膜早破组产妇与足月胎膜早破组产妇血清ZO-1、CCL2水平差异无统计学意义(P>0.05);早产胎膜早破组、足月胎膜早破组、对照组发生宫内感染的例数分别为39、23、2例,三组之间比较差异有统计学意义(χ^(2)=43.820,P<0.001);与未感染组比较,合并宫内感染胎膜早破产妇血清ZO-1水平显著降低,CCL2水平显著升高(P<0.05);血清ZO-1、CCL2联合预测胎膜早破合并宫内感染的曲线下面积为0.934,敏感度为93.55%,特异度为75.95%。结论胎膜早破患者血清ZO-1水平降低,CCL2水平升高,与患者宫内感染相关,血清ZO-1、CCL2水平对胎膜早破合并宫内感染有一定诊断价值。Objective To investigate the diagnostic value of serum zonule occluden-1(ZO-1)and CC chemokine ligand 2(CCL2)in premature rupture of membranes complicated with intrauterine infection.Methods A total of 141 women with premature rupture of membranes who were admitted to our hospital from September 2020 to March 2022 were taken as observation objects.According to whether the fetus was full term,the patients were divided into the preterm premature rupture of membranes group of 73 cases and the full-term premature rupture of membranes group of 68 cases.Patients were divided into infected group and uninfected group according to whether there was intrauterine infection or not.Another 70 women with full-term normal pregnancy and singleton were taken as the control group.The expression levels of serum ZO-1 and CCL2 were detected by enzyme-linked immunosorbent assay(ELISA),and ROC curve was used to analyze the diagnostic value of serum ZO-1 and CCL2 for premature rupture of membranes complicated with intrauterine infection.Results There were obvious differences in gestational age among the preterm premature rupture of membranes group,the full-term premature rupture of membranes group and the control group(P<0.05),there was no obvious difference in maternal age,pre-pregnancy BMI and proportion of pregnant women among the groups(P>0.05);compared with the control group,the level of serum ZO-1 in the preterm premature rupture of membranes group and the full-term premature rupture of membranes group decreased,and the level of CCL2 increased obviously(P<0.05),there was no obvious difference in the levels of serum ZO-1 and CCL2 levels between the preterm premature rupture of membranes group and the full-term premature rupture of membranes group(P>0.05);the number of cases of intrauterine infection in the premature rupture of membranes group,full-term premature rupture of membranes group,and control group was 39,23,and 2,respectively,and there was a statistically obvious difference among the three groups(χ^(2)=43.820,P<0.001);com
