腹腔镜手术后应用米非司酮治疗子宫内膜异位症的疗效观察

摘　　要：目的分析子宫内膜异位症患者腹腔镜手术后应用米非司酮的临床疗效。方法110例子宫内膜异位症患者,随机分为观察组与对照组,每组55例。两组患者均采用腹腔镜手术治疗,术后,观察组患者服用米非司酮治疗,对照组患者服用孕三烯酮治疗。对比两组患者痛经缓解情况、不良反应发生情况、排卵恢复时间、月经恢复时间及随访2年累计妊娠率。结果两组患者痛经缓解率对比差异无统计学意义(P>0.05)。观察组患者不良反应发生率为23.6%,明显低于对照组的45.5%,差异具有统计学意义(P<0.05)。观察组患者排卵恢复时间(12.79±3.12)d、月经恢复时间(24.89±2.13)d明显短于对照组的(25.64±5.01)、(38.07±5.04)d,差异具有统计学意义(P<0.05);两组患者随访2年累计妊娠率比较差异无统计学意义(P>0.05)。结论子宫内膜异位症患者腹腔镜手术后应用米非司酮具有较好的治疗效果,排卵及月经恢复时间短,不良反应少,安全性高,建议临床推广应用。Objective To analyze the clinical efficacy of mifepristone in the treatment of endometriosis after laparoscopic surgery.Methods A total of 110 patients with endometriosis were randomly divided into an observation group and a control group,with 55 cases in each group.Patients in both groups were treated by laparoscopic surgery.After surgery,patients in the observation group were treated with mifepristone,and patients in the control group were treated with gestrinone.Both groups were compared in terms of relief of dysmenorrhea,occurrence of adverse reactions,recovery time of ovulation,recovery time of menstruation and cumulative pregnancy rate at 2 years of follow-up.Results There was no statistically significant difference in remission rate of dysmenorrhea between the two groups(P>0.05).The incidence of adverse reactions of patients in the observation group was 23.6%,which was significantly lower than 45.5%in the control group,and the difference was statistically significant(P<0.05).The recovery time of ovulation and menstruation were(12.79±3.12)and(24.89±2.13)d in the observation group,which were significantly shorter than(25.64±5.01)and(38.07±5.04)d in the control group,and the differences were statistically significant(P<0.05).There was no statistically significant difference in the cumulative pregnancy rate between the two groups at 2 years of follow-up(P>0.05).Conclusion The application of mifepristone after laparoscopic surgery in patients with endometriosis has a better therapeutic effect,short recovery time of ovulation and menstrual,few adverse reactions and high safety,and is recommended for clinical promotion.
