摘  要:目的:研究腺样体肥大(adenoid hypertrophy,AH)患儿在缺氧状态下变应性鼻炎(allergic rhinitis,AR)免疫反应加重的原因,并对其机制进行探讨。方法:选择2018年1月-2020年12月佛山市第一人民医院鼻科诊治的100例AH患儿,根据是否伴有AR分将患儿分为AH组40例(单纯性AH患儿),AH合并AR组60例(AH合并AR患儿),同期选择对照组50例(健康对照儿童)。采用ELISA法检测上述三组血清和鼻黏膜低氧诱导因子-1α(hypoxia inducible factor-1,HIF-1α)和Th2相关因子[白细胞介素-4(IL-4)、白细胞介素-5(IL-5)和白细胞介素-17(IL-17)]表达;采用流式细胞术检测外周血树突状细胞(dendritic cell,DC)[白细胞介素-17受体B(IL-17RB)、白细胞介素-33及其受体(ST2)、胸腺基质淋巴细胞生成素的受体(TSLPR)]的表达;采用Pearson分析HIF-1α与Th2相关因子的相关性。结果:与对照组比较,AH合并AR组、AH组血液和鼻黏膜HIF-1α表达均明显升高,而AH合并AR组升高均更为明显(P<0.05)。与对照组比较,AH合并AR组、AH组DC中IL-17RB、ST2和TSLPR的表达均明显升高,且AH合并AR组均高于AH组(P<0.05)。与对照组比较,AH合并AR组、AH组血液和鼻黏膜Th2相关因子(IL-4、IL-5和IL-17)表达均明显升高,而AH合并AR组升高均更为明显(P<0.05)。AH合并AR组血清和鼻黏膜HIF-1α水平与IL-4、IL-5和IL-17均呈正相关(P<0.05)。结论:缺氧状态下AH患儿机体内通过激活DC功能,诱发AR患者Th2相关因子表达升高,激活机体的免疫反应,导致AR加重。Objective:To investigate the causes of allergic rhinitis(AR)increased immune reaction in children with adenoid hypertrophy(AH)under hypoxia,and to explore the mechanism.Method:A total of 100 AH children diagnosed and treated in the Nasal Department of the First People’s Hospital of Foshan from January 2018to December 2020 were selected,according to whether the children were accompanied by AR,the children were divided into 40 cases of AH group(simple AH children),60 cases of AH combined with AR group(AH combined with AR children),and 50 cases of control group(healthy control children)during the same period were selected.The expression of hypoxia inducible factor-1(HIF-1α)and Th2-related factors[interleukin-4(IL-4),interleukin-5(IL-5)and interleukin-17(IL-17)]in serum and nasal mucosa of the three groups were detected by ELISA method;the expression of peripheral blood dendritic cells(DC)[interleukin-17 receptor B(IL-17RB),interleukin-33 and its receptor(ST2),and thymic stromal lymphopoietin receptor(TSLPR)]were determined by flow cytometry;Pearson correlation was used to analyze the correlation between HIF-1αand Th2-related factors.Result:Compared with the control group,HIF-1αexpression in blood and nasal mucosa of AH combined with AR group and AH group were significantly increased,and they were more significantly increased in AH combined with AR group(P<0.05).Compared with the control group,the expressions of IL-17RB,ST2 and TSLPR in AH combined with AR group and AH group were significantly higher,they were more significantly increased in AH combined with AR group(P<0.05).Compared with the control group,the expression of Th2-related factors(IL-4,IL-5 and IL-17)in blood and nasal mucosa of AH combined with AR group and AH group were significantly increased,and the increase were more obvious in AH combined with AR group(P<0.05).The serum and nasal mucosa HIF-1αlevels of AH combined with AR group were positively correlated with IL-4,IL-5 and IL-17(P<0.05).Conclusion:The activation of DC function in AH children
