摘 要:IgA肾病是目前世界范围内最常见的原发性肾小球疾病,主要表现为肾小球系膜区IgA沉积或以IgA沉积为主。越来越多的证据表明,肠黏膜屏障的改变与IgA肾病的发生、发展有着密切的关联。明确肠黏膜屏障稳态的维持以及肠黏膜屏障破坏的机制对研究IgA肾病的发生、发展及治疗有着重要作用。本文综述了肠黏膜屏障改变影响IgA肾病的机制以及重塑肠道微环境、改善肠黏膜屏障、调节肠道内免疫细胞的增殖、分化和分泌对改善和治疗IgA肾病的作用。IgA nephropathy is the most common primary glomerular disease in the world.It is mainly manifested as IgA deposition or mainly IgA deposition in glomerular mesangium.Growing evidences show that the changes of intestinal mucosal barrier are closely correlated with the occurrence and development of IgA nephropathy.Clarifying the mechanism of maintenance of intestinal mucosal barrier homeostasis and the destruction of intestinal mucosal barrier plays an important role in researches on the occurrence,development and treatment of IgA nephropathy.This review summarized the mechanism of intestinal mucosal barrier changes affecting IgA nephropathy and the effects of remodeling intestinal microenvironment,improving intestinal mucosal barrier and regulating proliferation,differentiation and secretion of immune cells in intestinal tract in improvement and treatment of IgA nephropathy.
