地佐辛复合不同剂量右美托咪定对功能性鼻内窥镜手术患者氧化应激的影响研究

摘　　要：目的探讨地佐辛联合不同剂量右美托咪定在功能性鼻内窥镜手术患者中应用对机体氧化应激反应产生的影响.方法将2020年3月-2022年4月期间进入江南大学附属医院接受功能性鼻内窥镜手术治疗的84例患者采用计算机随机分组法进行分组,共分为四组(A组、B组、C组、D组).术中所有患者均行气管内插管全麻,且A组、B组、C组分别使用低(0.5μg/kg)、中(0.75μg/kg)、高(1.0μg/kg)剂量的右美托咪定复合地佐辛辅助全麻.观察各组患者氧化应激指标、心率等变化情况及镇静、镇痛效果.结果A组、B组、C组三组术后1d、3d的SOD水平均显著高于D组,MDA水平均显著低于D组(P<0.05),A组、B组、C组术后1d、3d的SOD、MDA水平比较均无显著差异(P>0.05);A组、B组、C组T0、T1、T2时平均动脉压、心率相比D组均显著更低(P<0.05),但A组、B组、C组组间比较无显著差异(P>0.05);四组术后平均拔管、苏醒时间对比均无显著差异(P>0.05),但与D组相比,A组、B组、C组苏醒期躁动发生率显著更低,镇痛满意度评分显著更高(P<0.05),A组、B组、C组组间比较无显著差异(P>0.05).结论地佐辛复合不同剂量右美托咪定辅助麻醉方式用于功能性鼻内窥镜手术患者,均可明显减轻患者氧化应激反应,且提高镇静、镇痛效果,不会增加麻醉并发症发生率,具备良好的有效性和安全性.Objective To investigate the effect of dezocine combined with different doses of dexmedetomidine on oxidative stress response in patients with functional nasal endoscopic surgery.Methods The 84 patients enrolled in our hospital from March 2020 to April 2022 were randomly divided into 4 groups(groups A,B,C and D).All patients underwent endotracheal general anesthesia,with low(0.5μg/kg),medium(0.75μg/kg),and high(1.0μg/kg)doses in groups A,B and C,respectively.Changes in oxidative stress index,heart rate and the effect of sedation and analgesia were observed.Results The postoperative SOD levels were significantly higher in groups A,B and C than in group D,the MDA levels were all significantly lower than that in group D(P<O.05),no significant differences in SOD and MDA levels at ld and 3d were observed in groups A,B and C(P>0.05);mean arterial pressure and heart rate in groups A,B,C,T0,T1 and T2 were significantly lower compared to group D(P<0.05),however,groups A,B and C will have no significant difference(P>0.05);the mean extubation and wake time in the 4 groups(P>0.05),but when compared to Group D,the incidence of waking agitation was significantly lower in groups A,B and C,analgesia satisfaction score was significantly higher(P<0.05),there were no significant difference between groups A,B and C(P>0.05).Conclusion Dezocine+different doses of dexmedetomidine to support anesthesia in patients with functional nasal endoscopic surgery,can significantly reduce the oxidative stress in patients,and improve the sedative and analgesic effect,will not increase the anesthesia complications,with good effectiveness and safety.
