可溶性髓样细胞触发受体-1和白介素-10在烧伤脓毒血症诊断中的应用价值

摘　　要：目的探讨可溶性髓样细胞触发受体-1(soluble myeloid cell trigger receptor-1,sTREM-1)及白介素-10(Interleukin-10,IL-10)水平变化与烧伤脓毒血症患者预后的相关性。方法选取2019年10月—2022年8月邢台市人民医院烧伤科收治的150例烧伤脓毒血症患者为观察组,另将邢台市人民医院同期健康体检者150例纳为对照组。分别检测治疗第1、3、5、7天血清sTREM-1和IL-10水平,根据预后结局分为好转组和恶化组,比较两组血清sTREM-1和IL-10水平,并进行二者与患者预后的Spearman相关分析。结果烧伤脓毒血症组患者第1、3、5、7天血清sTREM-1和IL-10水平均明显高于对照组(P<0.05);恶化组第1、3、5、7天血清sTREM-1和IL-10水平均明显高于好转组(P<0.05)。Spearman相关分析显示,烧伤脓毒血症患者血清sTREM-1及IL-10总体水平与患者预后均呈正相关(r=0.487,r=0.529)。结论烧伤脓毒血症患者sTREM-1与IL-10水平随病情变化而动态改变,可反映早期烧伤脓毒血症患者病情。sTREM-1与IL-10水平对烧伤脓毒血症患者病情预后有良好的预示作用。Objective To investigate the relationship between the changes of soluble myeloid cell trigger receptor-1(sTREM-1)and IL-10 and the prognosis of burn patients with sepsis.Methods From October 2019 to August 2022,a total of 150 patients with burn sepsis of Xingtai People’s Hospital were selected as observation group,and a total of 150 health examinees in the same period of Xingtai People’s Hospital as the control group,the serum levels of sTREM-1 and IL-10 were detected on day 1,3,5 and 7 of treatment respectively,according to the prognosis,the patients were divided into improved group and deteriorated group.The serum levels of sTREM-1 and IL-10 were compared between the two groups.Results The level of serum sTREM-1 and IL-10 in burn sepsis group was significantly higher than that in control group(P<0.05),and the level of sTREM-1 and IL-10 in deteriorated group was significantly higher than that in improved group(P<0.05).The results of Spearman correlation analysis showed that there was a positive correlation between the total serum levels of sTREM-1 and IL-10 and the prognosis of burn sepsis(r=0.487,r=0.529).Conclusion The dynamic changes of STREM-1 and IL-10 in burn sepsis patients were related to the severity of burn sepsis.The levels of sTREM-1 and IL10 can predict the prognosis of burn patients with sepsis.
