个体化阶梯性药物治疗控制变应性鼻炎症状的优势评估

摘　　要：目的评价个体化阶梯性药物治疗控制常年性变应性鼻炎(AR)患者症状的效果及优势。方法选择在我院专病门诊接受个体化阶梯药物治疗的AR患者作为观察组,随机抽取同期普通门诊的AR患者作为对照组。在治疗初始、2周、1个月、3个月、6个月、9个月、12个月采用视觉模拟量表(VAS)评分、鼻部症状总评分(TNSS)、鼻腔-鼻窦结局测试(SNOT-20),鼻炎控制评分测试(RCAT)进行评估。采用SPSS 24.0统计学软件对相关数据进行统计分析。结果观察组和对照组的VAS、TNSS、SNOT-20、RCAT评分在初始和治疗后1个月时差异均无统计学意义(P值均>0.05);1个月后,2组指标逐渐拉开差距,到第6个月时差距最大。观察组在治疗后第3个月时,疗效达到最佳状态且保持稳定至随访结束。结论个体化阶梯性药物治疗能显著改善常年性AR患者的主观症状,尤其是鼻部相关症状,并且能在较长时间内保持稳定。Objective To evaluate the efficacy and advantages of individualized stepwise pharmacotherapy in controlling the symptoms of patients with perennial allergic rhinitis(AR).Methods Patients with AR who had received individualized stepwise pharmacotherapy in the Rhinitis Clinic of Minhang Hospital Affiliated to Fudan University were selected for the observation group.Additionally,patients with AR from the general clinic during the same period were randomly sorted to the control group.The visual analogue scale(VAS),total nasal symptom score(TNSS),sino-nasal outcome test 20(SNOT-20)and rhinitis control assessment test(RCAT)were used to evaluate the symptoms at initial,2 weeks,1 month,3 months,6 months,9 months,12 months after the treatment respectively.SPSS 24.0 statistic soft ware was used to analyze the data between the two groups.Results There were no significant differences in VAS,TNSS,SNOT-20 or RCAT scores between the two groups at the beginning and 1 month after the treatment.The differences in the above indexes between the two groups gradually widened after 1 month,and reached the maximum after 6 months.The stepwise pharmacotherapy group achieved the best efficacy at the end of the third month and maintained until the end point.Conclusions Individualized step-by-step drug strategy could significantly improve the subjective symptoms of patients with perennial AR,especially the nasal symptoms,and has a stable efficacy for a long time.
