双Endobutton柔性固定技术与锁骨钩钢板治疗TossyⅢ型肩锁关节脱位的近期疗效分析

摘　　要：目的比较双Endobutton技术与锁骨钩钢板(CHP)治疗TossyⅢ型肩锁关节脱位的近期疗效。方法回顾性分析收治的59例TossyⅢ型肩锁关节脱位患者资料,按治疗方式不同分为双Endobutton组(观察组)和CHP组(对照组)。对比分析2组围术期指标,疼痛视觉模拟评分(VAS)、Constant-Murley评分及Karlsson评分。结果观察组手术时间长于对照组,切口长度小于对照组,差异有统计学意义(P<0.05)。2组的术中出血量比较无统计学意义(P>0.05)。术后3天观察组VAS评分低于对照组(P<0.05)。2组患者术前及术后1周VAS评分对比无统计学意义(P>0.05)。观察组术后1周、术后1月、术后3月、术后6月Constant-Murley评分均低于对照组(P<0.05)。观察组术后6个月Karlsson评分,优良率为100.00%,对照组优良率为93.93%(P<0.05)。结论双Endobutton治疗肩锁关节脱位有利于早期肩关节全范围活动,可最大程度达到微创,术后瘢痕小,有较为理想的疗效。Objective To compare the short-term efficacy of double Endobutton flexible fixation technique and clavicular hook plate(CHP)in the treatment of TossyⅢacromioclavicular joint dislocation.Methods The data of 59 patients with TossyⅢacromioclavicular joint dislocation were retrospectively analyzed and divided into double Endobutton group(observation group)and CHP group(control group)according to the different treatment methods.The perioperative indicators,visual analogue scale(VAS),Constant-Murley score and Karlsson score of pain in the two groups were compared and analyzed.Results The operation time of the observation group was longer than that of the control group,and the length of the incision was shorter than that of the control group,and the difference was statistically significant(P<0.05).There was no significant difference in intraoperative bleeding between the two groups(P>0.05).The VAS score of the observation group was lower than that of the control group(P<0.05).There was no statistical significance in VAS scores between the two groups before and 1 week after surgery(P>0.05).The Constant-Murley scores of the observation group were lower than those of the control group at 1 week,1 month,3 months,and 6 months after operation(P<0.05).According to the Karlsson score at 6 months after operation,the excellent and good rate of the observation group was 100.00%,and the excellent and 93.93%in the control group,and the difference was statistically significant(P<0.05).Conclusion Double Endobutton for the treatment of acromioclavicular joint dislocation is beneficial to the early full range of shoulder joint movement,which can achieve minimally invasive with small postoperative scars,and has an ideal curative effect.
