SIRS评分联合系统性炎症指标对儿童RSV混合感染的诊断价值

摘　　要：目的探讨全身炎症反应综合征(SIRS)评分联合系统性炎症指标对儿童呼吸道合胞病毒(RSV)混合感染的诊断价值。方法RSV感染患儿175例分为RSV混合感染组(A组,65例)和RSV单纯感染组(B组,110例)。比较两组SIRS评分和系统性炎症指标。绘制ROC曲线分析SIRS评分联合系统性炎症指标对儿童RSV混合感染的诊断效能。结果与B组比较,A组SIRS评分、IL-6水平、中性粒细胞计数、中性粒细胞计数/淋巴细胞计数比值(NLR)、血小板计数/淋巴细胞计数比值(PLR)及系统性免疫炎症指数(SII)均升高(P<0.05)。SIRS评分联合IL-6、联合中性粒细胞计数、联合NLR、联合PLR、联合SII诊断儿童RSV混合感染的AUC分别为0.762、0.817、0.822、0.811、0.824(P<0.01);其中,SIRS评分联合SII诊断儿童RSV混合感染的灵敏度和特异度最高,分别为74.31%和83.82%。结论SIRS评分联合系统性炎症指标能够提高儿童RSV混合感染的诊断效能,其中SIRS评分联合SII对儿童RSV混合感染的诊断价值最高。Objective To explore diagnostic value of systemic inflammatory response syndrome(SIRS)score combined with systemic inflammatory indicators for mixed respiratory syncytial virus(RSV)infection in children.Methods A total of 175 children with mixed RSV infection were assigned into two groups of A(with mixed RSV infection,65 cases)and B(with RSV infection only,110 cases).The SIRS score and systemic inflammatory indicators were compared between the two groups.The ROC curve was drawn to analyze the diagnostic value of SIRS score combined with systemic inflammatory indicators in the children with mixed RSV infection.Results Compared with group B,the SIRS score,neutrophil count,IL-6 level,neutrophil count/lymphocyte count ratio(NLR),platelet count/lymphocyte count ratio(PLR)and systemic immune-inflammation index(SII)were higher in group A(P<0.05).The AUC of SIRS score combined with each of IL-6,neutrophil count,NLR,PLR and SII in the diagnosis of mixed RSV infection in children was 0.762,0.817,0.822,0.811 and 0.824,respectively(P<0.01).SIRS score combined with SII had the highest sensitivity and specificity,which were 74.31% and 83.82%,respectively.Conclusion The SIRS score combined with systemic inflammatory indicators can improve the diagnostic efficacy of mixed RSV infection in children,in which the SIRS score combined with SII has the highest diagnostic value.
