糖尿病肾病与致炎因子的相关性及益气养阴、祛瘀化浊法干预后的影响

摘　　要：目的探讨糖尿病肾病与致炎因子的相关性及益气养阴、祛瘀化浊法干预后的影响。方法选取我院肾病科收治的60例气阴两虚、瘀浊阻络型糖尿病肾病(Ⅲ-Ⅳ期)患者,随机分为对照组及治疗组各30例。对照组予西医基础治疗,如糖尿病肾病饮食等;治疗组在对照组基础上予具有益气养阴、祛瘀化浊功效的平糖益肾方加减治疗,比较两组用药前后实验室指标、中医证候疗效、致炎因子水平,进一步行Spearman相关性分析。结果治疗后治疗组24 h-Upro、UACR、Scr、FBG、HbA1c均低于对照组,差异有统计学意义。两组致炎因子NF-кB;C5b-9;IL-6水平均低于治疗前,且治疗组的致炎因子水平明显低于对照组。24 h-Upro、Scr、HbA1c、BMI与致炎因子有一定相关性。结论糖尿病肾病与致炎因子具有一定的相关性。具有益气养阴、祛瘀化浊功效的平糖益肾方可改善糖尿病肾病患者的临床症状,延缓疾病的发展,可能与下调致炎因子水平,降低炎症反应有关。Methods 60 cases of diabetic nephropathy(stageⅢ-Ⅳ)with Qi Yin deficiency and blood stasis obstruction were selected and randomly divided into the control group and the treatment group,30 cases in each group.The control group was treated with western medicine,such as diabetic nephropathy and diet.The treatment group was treated with Pingtang Yishen formula on the basis of the control group.The laboratory indexes,TCM syndrome curative effect and proinflammatory factor level of the two groups before and after treatment were compared,and the Spearman correlation analysis was further performed.Results 24 hours after treatment,the levels of Upro,UACR,SCR,FBG and HbA1c in the treatment group were significantly lower than those in the control group.The levels of NF-кB,C5b-9 and IL-6 in the two groups were lower than those before treatment,and the levels of proinflammatory factors in the treatment group were significantly lower than those in the control group.24 h Upro,SCR,HbA1c and BMI were correlated with proinflammatory factors.Conclusion Diabetic nephropathy has a certain correlation with proinflammatory factors.Ping Tang Yishen Decoction can improve the clinical symptoms of diabetic nephropathy and delay the development of the disease,which may be related to the reduction of the level of proinflammatory factors and the reduction of inflammatory reaction.objective To investigate the correlation between diabetic nephropathy and inflammatory factors and the effects of Qi supplementing,Yin nourishing and removing blood stasis and turbidity on the prognosis.
