摘 要:目的探讨胎儿超声心动图肺静脉血流频谱评估妊娠糖尿病(GDM)对胎儿左心室舒张功能影响的价值。方法抽取2020年1月至2021年12月黄河水利委员会黄河中心医院收治的66例GDM孕妇作为研究对象(GDM组),另随机抽取同期产检的66例正常孕妇作为健康组。所有对象均行胎儿超声心动图肺静脉血流频谱检测。比较两组胎儿肺静脉血流频谱参数、传统超声参数、血糖水平,采用Pearson相关性分析研究A峰流速与肺静脉搏动指数(PI)及肺静脉峰值流速指数(PVIV)的相关性,A峰流速、PI、PVIV与空腹血糖(FPG)、餐后2 h血糖(2hPG)的关系。结果GDM组FPG、2hPG水平高于健康组(P<0.05)。GDM组A峰流速低于健康组(P<0.05),PI、PVIV高于健康组(P<0.05)。Pearson相关性分析结果显示,A峰流速与PI、PVIV关系均呈负相关(r=-0.57、-0.46,P均<0.001)。两组胎儿二尖瓣环超声组织多普勒成像参数(舒张早期峰值速度、舒张晚期峰值速度和收缩期峰值速度)、二尖瓣血流频谱(二尖瓣口舒张早期峰值流速和舒张晚期峰值流速、Tei指数)比较差异未见统计学意义(P>0.05)。A峰流速与FPG、2hPG均呈负相关(r=-0.50,-0.48,P均<0.001),PI与FPG、2hPG均呈正相关(r=0.47、0.47,P均<0.001),PVIV与FPG、2hPG均呈正相关(r=0.46、0.46,P均<0.001)。结论GDM孕妇血糖控制水平影响胎儿左心室舒张功能,血糖控制越差胎儿左心室舒张功能越差,临床可通过胎儿超声心动图肺静脉血流频谱胎儿左心室舒张功能综合评估。Objective To investigate the value of echocardiographic fetal pulmonary vein blood flow spectrum in evaluation on influence of gestational diabetes mellitus(GDM)on infant left ventricular diastolic function.Methods Sixty-six pregnant women with GDM admitted to Yellow River Central Hospital of Yellow River Water Conservancy Commission from January 2020 to December 2021 were enrolled as the research objects(GDM group).Another 66 health pregnant women undergoing routine obstetric examinations during the same period were enrolled as healthy group.All selected pregnancy women underwent echocardiographic fetal pulmonary vein blood flow spectrum examination.The parameters of pulmonary venous blood flow spectrum,traditional ultrasound parameters and blood glucose level were compared between the two groups.The relationship of A peak velocity with pulsatility index(PI)and peak velocity index for veins(PVIV),and relationship of A peak velocity,PI and PVIV with fasting blood glucose(FPG)and 2 hours postprandial blood glucose(2hPG)were analyzed by Pearson correlation analysis.Results The levels of FPG and 2hPG in the GDM group were higher than those in the healthy group(P<0.05).A peak velocity in the GDM group was lower than that in the healthy group(P<0.05),while PI and PVIV were higher than those in the healthy group(P<0.05).Results of Pearson correlation analysis showed that A peak velocity was negatively correlated with PI and PVIV(r=-0.57,-0.46;both P<0.001).There was no significant difference between the two groups in mitral annulus tissue Doppler imaging parameters including early diastolic peak velocity,late diastolic peak velocity,systolic peak velocity,or mitral valve blood flow spectrum including early diastolic peak velocity of mitral valve orifice,late-diastolic peak velocity of mitral valve orifice and Tei index(P>0.05).A peak velocity was negatively correlated with FPG and 2hPG(r=-0.50,-0.48;both P<0.001),PI was positively correlated with FPG and 2hPG(r=0.47,0.47;both P<0.001),and PVIV was positively correlated
