摘 要:目的:探讨老年2型糖尿病患者肠道菌群数目及与血清C-肽(C-P)、胰岛素(Ins)水平的相关性,以期为疾病防治提供更多指导。方法:回顾性选取2021年3月—2022年1月高邮市中医医院诊治的56例老年2型糖尿病患者为观察组,另选取本院同期接受体检的56例健康老人为对照组。检测并比较两组血糖水平、肠道主要菌群数目、血清C-P、Ins水平,以Pearson相关系数分析肠道菌群数目与血清C-P、Ins水平的相关性。结果:观察组空腹血糖(FBG)、糖化血红蛋白(HbA1c)、餐后2 h血糖(2 h PG)水平明显高于对照组,差异有统计学意义(P<0.05);观察组肠道双歧杆菌、乳杆菌数目明显少于对照组,拟杆菌、肠球菌、肠杆菌、酵母菌数目明显多于对照组,差异有统计学意义(P<0.05);观察组血清C-P水平低于对照组,Ins水平高于对照组,差异有统计学意义(P<0.05);Pearson相关系数分析显示,双歧杆菌、乳杆菌数目与血清C-P水平呈正相关,与血清Ins水平呈负相关,差异有统计学意义(P<0.05),拟杆菌、肠球菌、肠杆菌、酵母菌数目与血清C-P水平呈负相关,与血清Ins水平呈正相关,差异有统计学意义(P<0.05)。结论:老年2型糖尿病患者肠道有益菌数目明显下降,条件致病菌数目明显上升,且肠道菌群数目与血清C-P、Ins水平有相关性。Objective: To explore the correlation between the number of intestinal flora and the levels of serum C-peptide(C-P) and insulin(Ins) in elderly patients with type 2 diabetes, so as to provide more guidance for disease prevention and treatment. Method: Retrospectively select 56 elderly patients with type 2 diabetes diagnosed and treated in Gaoyou Traditional Chinese Medicine Hospital from March 2021 to January 2022 as the observation group, and other 56 healthy elderly patients undergoing physical examination at the same time in our hospital as the control group. The blood glucose level, the number of main intestinal flora, the serum C-P and Ins levels were measured and compared between the two groups. The correlation between the number of intestinal flora and the serum C-P and Ins levels were analyzed by Pearson correlation coefficient. Result: The levels of fasting blood glucose(FBG), glycosylated hemoglobin(HbA1c) and 2 h postprandial blood glucose(2 h PG) in the observation group were significantly higher than those in the control group, and the differences were statistically significant(P<0.05). The number of intestinal Bifidobacteria and Lactobacillus in the observation group were significantly less than those in the control group, and the number of Bacteroides, Enterococcus, Enterobacter and Yeast in the observation group were significantly more than those in the control group, the differences were statistically significant(P<0.05).The level of serum C-P in the observation group was lower than that in the control group, and the level of Ins was higher than that in the control group, the differences were statistically significant(P<0.05). Pearson correlation coefficient analysis showed that the number of Bifidobacteria and Lactobacillus was positively correlated with the serum C-P level and negatively correlated with the serum Ins level,the differences were statistically significant(P<0.05).
