摘　　要：目的探讨血清抗米勒管激素(AMH)和抑制素B(InhB)联合检测在中枢性性早熟(CPP)男性儿童中的诊断价值。方法选取2015年12月至2018年12月江西省儿童医院收治的33例CPP男性患儿作为CPP组,选取同期30例青春发育前健康体检男童为对照组。测定两组的血清黄体生成素(LH)、卵泡刺激素(FSH)、AMH、InhB等指标。采用ROC曲线分析AMH、InhB水平诊断男性CPP的效能。结果CPP组患儿的骨龄、睾丸容积均大于对照组,血清AMH水平低于对照组,InhB及LH水平高于对照组,差异有统计学意义(P<0.05)。血清AMH与LH峰值(r=-0.579)、FSH基值(r=-0.381)、FSH峰值(r=-0.446)、睾丸容积(r=-0.632)均呈负相关(P<0.05),InhB仅与睾丸容积呈正相关(r=0.644,P<0.05)。ROC曲线预测InhB对CPP诊断的截断值为95.22 pg/ml,AMH对CPP诊断的截断值为38.64 ng/ml。结论血清AMH及InhB可能作为临床判断CPP男孩性腺轴启动的参考指标。Objective To investigate the diagnostic value of serum anti-Müllerian hormone(AMH)and inhibin B(InhB)in male children with central precocious puberty(CPP).Methods Thirty-three male children with CPP admitted to Jiangxi Provincial Children's Hospital from December 2015 to December 2018 were selected as the CPP group,and 30 healthy boys with physical examination before puberty were selected as the control group during the same period.Serum luteinizing hormone(LH),follicle stimulating hormone(FSH),AMH,InhB and other indexes of the two groups were determined.ROC curve was used to analyze the diagnostic efficacy of AMH and InhB levels in male CPP.Results Bone age and testicular volume of children in CPP group were higher than those in control group,serum AMH levels were lower than those in control group,and InhB and LH levels were higher than those in control group,the differences were statistically significant(P<0.05).Serum AMH was negatively correlated with the peak value of LH(r=-0.579),the basal value of FSH(r=-0.381),the peak value of FSH(r=-0.446)and testicular volume(r=-0.632)(P<0.05),while InhB was only positively correlated with testicular volume(r=0.644,P<0.05).According to ROC curve,the cut-off value of InhB for CPP diagnosis was 95.22 pg/ml,and that of AMH for CPP diagnosis was 38.64 ng/ml.Conclusion Serum AMH and InhB may be used as reference indexes to judge the initiation of gonadal axis in boys with CPP.
