摘 要:目的研究门冬胰岛素治疗妊娠期糖尿病(GDM)的疗效及对患者血糖控制和母婴结局的影响。方法选取40例GDM患者,根据随机数字表法分为生物组与门冬组,每组20例。生物组予以生物合成人胰岛素治疗,门冬组予以门冬胰岛素治疗。对比两组血糖水平变化及母婴结局。结果治疗前,两组空腹血糖(FPG)、餐后2 h血糖(2h PG)及糖化血红蛋白(HbA1c)水平比较,差异无统计学意义(P>0.05);治疗后,两组FPG、2h PG及HbA1c水平均明显低于治疗前,且门冬组明显低于生物组,差异有统计学意义(P<0.05)。门冬组顺产率85.00%高于生物组的50.00%,分娩孕周(39.20±1.10)周长于生物组的(37.98±1.12)周,不良妊娠结局发生率30.00%低于生物组的65.00%,差异有统计学意义(P<0.05)。结论门冬胰岛素治疗GDM可有效控制血糖,安全性良好,改善母婴结局。Objective To study the effect of insulin aspartate on gestational diabetes mellitus(GDM)blood glucose control and maternal and infant outcomes.Methods 40 patients with GDM were selected and divided into biological group and aspartate group according to random number table method,with 20 cases in each group.The biological group was treated with biosynthetic human insulin and the aspartate group was treated with aspartate insulin.The changes of blood glucose level and maternal and infant outcomes were compared between the two groups.Results Before treatment,there were no significant differences in fasting blood glucose(FPG),2h postprandial blood glucose(2h PG)and glycosylated hemoglobin(HbA1c)levels between the two groups(P>0.05).After treatment,FPG,2h PG and HbA1c levels in 2 groups were significantly lower than before treatment,and aspartate group was significantly lower than biological group,the difference was statistically significant(P<0.05).The rate of vaginal delivery in aspartate group was 85.00%higher than that in biological group(50.00%),and the circumference of gestational week(39.20±1.10)was higher than that in biological group(37.98±1.12).The incidence of adverse pregnancy outcome was 30.00%lower than that in biological group(65.00%),and the difference was statistically significant(P<0.05).Conclusion Insulin aspartate in the treatment of GDM can effectively control blood sugar with good safety and improve maternal and infant outcomes.
