摘　　要：目的 探讨超敏C-反应蛋白(hs-CRP)、白细胞介素-6(IL-6)、白细胞(WBC)计数检测对小儿呼吸道病毒感染的诊断价值。方法 选取2020年9月至2022年2月收治的50例小儿呼吸道感染患儿为研究对象(病毒感染组),另选50例同期健康儿童为对照组。比较2组hs-CRP、IL-6、WBC水平及一般资料,分析小儿呼吸道病毒感染危险因素,并绘制ROC曲线。结果 病毒感染组患儿hs-CRP、IL-6、D-二聚体、降钙素原(PCT)、WBC、血清淀粉样蛋白(SAA)水平显著高于对照组(P<0.05);病毒感染组居住农村、有早产史、哮喘史患儿占比显著高于对照组(P<0.05);小儿呼吸道病毒感染时hs-CRP、IL-6、WBC水平升高,且居住农村、有早产史及哮喘史是小儿呼吸道病毒感染的危险因素(P<0.05);hs-CRP、IL-6、WBC及联合诊断小儿呼吸道病毒感染的曲线下面积AUC分别为0.644、0.885、0.617、0.923,联合诊断AUC显著高于单独检测(P<0.05)。结论 hs-CRP、IL-6、WBC在小儿呼吸道病毒感染患儿中水平较高,联合检测诊断价值较高。Objective To investigate the diagnostic value of ultrasensitive C-reactive protein(hs-CRP),interleukin-6(IL-6),and white blood cell(WBC) count for pediatric respiratory viral infections.Methods Fifty children with respiratory infections admitted to our hospital from September 2020 to February 2022 were recruited in the viral infection group,and another 50 healthy children from the same period were selected as the control group.hs-CRP,IL-6 and WBC levels,and baseline characteristics of the 2 groups were compared.Risk factors of pediatric respiratory viral infections were analyzed and ROC curves were drawn.Results hs-CRP,IL-6,D-dimer,calcitoninogen(PCT),WBC,and serum amyloid(SAA) were significantly higher in children in the viral infection group than those in the control group(P<0.05) The proportion of children living in rural areas,with a history of prematurity and asthma was significantly higher in the viral infection group than that in the control group(P<0.05).Rural residence,history of prematurity,asthma,and higher levels of hs-CRP,IL-6,and WBC were risk factors for pediatric respiratory viral infections(P<0.05).The area under the curve AUCs for hs-CRP,IL-6,WBC and their combination in diagnosing pediatric respiratory viral infection was 0.644,0.885,0.617 and 0.923,respectively.AUC of the combination detection was significantly higher than that of a single detection(P<0.05).Conclusion hs-CRP,IL-6,and WBC are higher levels in diagnosing pediatric respiratory viral infections,and their combination detection is a top diagnostic value.
