摘 要:目的:探讨个体化饮食对腹膜透析患者营养状况及钙磷代谢的影响。方法:选取2020年1月1日~2021年12月31日收治的90例腹膜透析患者作为研究对象。按照1∶1形式将患者分为对照组和观察组各45例,对照组采用腹膜透析常规饮食护理,观察组采用个体化饮食干预;比较两组干预前后营养指标[包括血清白蛋白(ALB)、转铁蛋白(SF)、体质量指数(BMI)]和钙磷代谢、营养状态[采用主观综合营养评估(SGA)]及生活质量[采用生活质量综合评定量表(GQOLI-74)]。结果:干预后,观察组ALB高于干预前和对照组(P<0.01),对照组ALB低于干预前(P<0.01);干预后,两组SF、BMI均高于干预前(P<0.01),且观察组高于对照组(P<0.01);干预后,两组血磷、钙磷乘积SGA评分低于干预前(P<0.01),且观察组低于对照组(P<0.01,P<0.05);干预后,两组血钙高于干预前(P<0.01),但观察组低于对照组(P<0.01);干预后,两组GQOLI-74中心理功能、社会功能、物质生活、躯体功能评分均高于干预前(P<0.01),且观察组高于对照组(P<0.01,P<0.05)。结论:对腹膜透析患者给予个体化饮食干预,可提升患者营养情况,改善钙磷代谢,提高生活质量,有利于患者预后。Objective:To explore the effect of individualized diet on nutritional status,calcium and phosphorus metabolism in patients with peritoneal dialysis.Methods:A total of 90 patients with peritoneal dialysis from January 1,2019 to December 31,2020 were selected as the research objects 45 patients in each group were in the form of 1∶1 were divided into the control group and the observation group.The control group received routine diet nursing for peritoneal dialysis,and the observation group received individualized diet intervention.The nutritional status including serum albumin(ALB),transferrin(SF),body mass index(BMI),calcium and phosphorus metabolism,nutritional status by subjective comprehensive nutrition assessment(SGA)and quality of life scores by GQOLI-74 between the two groups were compared.Results:After the intervention,the levels of serum ALB in the observation group were higher than those before intervention and in the control group(P<0.01),and the levels of serum ALB in the control group were lower than those before intervention(P<0.01).After the intervention,the SF and BMI in both groups were higher than those before intervention(P<0.01),and those in the observation group were higher than those in the control group(P<0.01).The blood phosphorus,blood calcium and calcium phosphorus product scores were lower than those before intervention(P<0.01),and those in the observation group were lower than those in the control group(P<0.01,P<0.05).After the intervention,two groups of blood calcium is higher than before the intervention(P<0.01),but the observation group is lower than the control group(P<0.01).After intervention,the GQOLI-74 psychological function,social function,material life and physical function scores in both groups were higher than before the intervention(P<0.01),and those in the observation group were higher than those in the control group(P<0.01,P<0.05).Conclusion:Individualized diet intervention in peritoneal dialysis patients can improve their nutritional status,calcium and phosphorus metabol
