督灸配合右归丸治疗肾阳虚型围绝经期综合征疗效观察

摘　　要：目的观察督灸配合右归丸治疗肾阳虚型围绝经期综合征的临床疗效及其对患者性激素、骨密度和生活质量的影响。方法将124例肾阳虚型围绝经期综合征患者随机分为治疗组和对照组,每组62例。对照组采用右归丸加味煎汤内服治疗,治疗组在对照组基础上采用督灸治疗。观察两组治疗前后各项中医证候积分、Kupperman绝经指数(Kupperman menopausal index,KMI)评分、围绝经期生存质量量表(menopause-specific quality of life,MENQOL)评分、血清性激素[促黄体生成素(luteining hormone,LH)、促卵泡激素(folliclestimulating hormone,FSH)、雌二醇(estradiol,E2)]和抗缪勒管激素(anti-Müllerian hormone,AMH)水平及骨密度变化情况,并比较两组临床疗效及不良反应发生率。结果治疗组总有效率为91.9%,明显高于对照组的74.2%,两组比较差异具有统计学意义(P<0.05)。两组治疗后各项中医证候积分、KMI评分、MENQOL评分及血清FSH、LH、AMH水平均较同组治疗前显著下降,血清E2水平及骨密度均显著上升,差异均具有统计学意义(P<0.05)。治疗组治疗后各项中医证候积分、KMI评分、MENQOL评分及各项性激素、AMH水平、骨密度与对照组比较,差异均具有统计学意义(P<0.05)。两组治疗期间不良反应发生率比较,差异无统计学意义(P>0.05)。结论督灸配合右归丸是一种治疗肾阳虚型围绝经期综合征的有效方法,能提高血清E2水平,抑制血清FSH、LH、AMH水平,增加骨密度,提高患者生活质量。Objective To observe the clinical efficacy of Governor Vessel moxibustion plus Yougui Pill for perimenopausal syndrome of kidney-yang deficiency type and its effect on sex hormones,bone mineral density and the quality of life.Method One hundred and twenty-four patients with perimenopausal syndrome of kidney-yang deficiency type were randomized to treatment and control groups,with 62 cases in each group.The control group received oral administration of modified Yougui Pill decoction and the treatment group,Governor Vessel moxibustion in addition.The TCM symptom item scores,the Kupperman Menopausal Index(KMI)score,the menopause-specific quality of life(MENQOL)score,serum sex hormones(luteining hormone(LH),follicle-stimulating hormone(FSH),progesterone and estradiol(E2),anti-Müllerian hormone(AMH)levels and bone mineral density were observed in the two groups before and after treatment.The clinical therapeutic effects and the incidence of adverse reactions were compared between the two groups.Result The total efficacy rate was 91.9%in the treatment group,which was significantly higher than 74.2%in the control group with a statistically significant difference between the two groups(P<0.05).The TCM symptom item scores,the KMI score,the MENQOL score,and serum FSH,LH and AMH levels decreased significantly and serum E2 levels and bone mineral density increased significantly in the two groups after treatment compared with before,with statistically significant differences(P<0.05).Post-treatment TCM symptom item score,KMI score,MENQOL score had statistically significant differences between the treatment and control groups(P<0.05).The incidence of adverse reactions had no statistically significant differences between the two groups(P>0.05).Conclusion Dumeridian moxibustion plus Yougui Pill is an effective way to treat perimenopausal syndrome of kidney-yang deficiency type.It can raise serum E2 levels,reduce serum FSH,LH and AMH levels,increase bone mineral density and improve the patients’quality of life.
