帕金森病异动症的中医诊疗进展

摘　　要：异动症,即左旋多巴诱导的异动症(Levodopa-induced dyskinesia,LID)是一类常见的帕金森病(Parkinson’s disease,PD)中晚期并发症,具有发病率高和预后较差的特点。中医药在治疗LID方面表现出了独特优势。此文从中医药角度入手,通过总结近年来各学者的学术观点与临床经验,发现肝肾阴虚为LID发病的根本病机,而虚、痰、瘀、毒均是发生LID的病理基础。根据最新专家共识,此文将LID主要证型分为肝肾阴虚证、气血亏虚证、痰瘀动风证和阳虚动风证,并系统地回顾归纳了近年来中医治疗LID的临床研究。Levodopa-induced dyskinesia(LID) is a common complication in the middle and late stages of Parkinson’s disease,which has a high incidence and poor prognosis.Traditional Chinese medicine(TCM) has unique advantages in the treatment of LID.From the perspective of TCM,the viewpoints and clinical experiences of various scholars in recent years are summarized in this article.The fundamental pathogenesis of LID is the syndrome of yin deficiency of liver and kidney.It is worth mentioning that deficiency,phlegm,static blood and poison are the pathological basis of LID.According to the latest expert consensus,the main syndrome types of LID are divided into the syndrome of yin deficiency of liver and kidney,the syndrome of deficiency of both qi and blood,the syndrome of wind stirring due to phlegm-static blood,and the syndrome of stirring wind due to yang deficiency.Moreover,we systematically review and summarize the clinical studies on LID with TCM treatment in recent years.
