摘 要:目的 探讨MR在胎儿脊柱脊髓疾病诊断中的应用价值.方法 收集彩色多普勒超声提示胎儿脊柱脊髓异常且行MR检查的22例胎儿的影像学资料,与出生后检查、上级医院会诊或尸检结果对照.总结各类胎儿脊柱脊髓病变的MR影像特点,分析MR的诊断准确性及各序列的优势性.结果 22例胎儿经MR诊断:显性神经管闭合不全5例,单纯椎体发育畸形4例,骶管囊肿4例、1例误诊应为脊髓栓系,成熟性畸胎瘤3例、1例误诊应为未成熟性畸胎瘤,尾部退化综合征3例,脊髓栓系综合征2例,椎管内血管畸形伴出血1例.MR正确诊断20例,准确率为90.9%;Haste序列显示脊髓形态及圆锥位置最佳;磁敏感加权成像(SWI)显示椎体形态最佳.结论 MR可评估胎儿脊髓及脊柱椎体病变,为明确胎儿脊柱脊髓病变提供可靠的影像学依据.Objective To explore the diagnostic value of MR in fetal spinal cord diseases.MethodssThe imaging data of 22 cases whose fetal spinal cord abnormalities were revealed by color Doppler ultrasound and MR examinations performed were collected.Summarized the MR features of various fetal spinal cords diseases,compared with the results of postnatal examination,consultation,or autopsy in a superior hospital,analyzed the diagnostic accuracy of MR and the advantages of each sequence.Results Among the 22 fetuses,MR diagnosed:1 case was found with an intravertebral vascular malformation and hemorrhage;4 cases had sacral canal cysts and 1 case was misdiagnosed.It should be a tethered cord syndrome;2 cases were found with tethered cord syndrome;3 cases were found with mature teratoma;1 case was misdiagnosed,it should be immature teratoma;3 cases were found with tail degenerative syndrome;5 cases were found with dominant neural tube insufficiency;4 cases were found with simple vertebral malformation;MR correctly diagnosed 20 cases(accuracy 90.9%);the Haste sequence was the best for showing the spinal cord shape and cone position;susceptibility weighted imaging(SWI)was the best for showing vertebral body shape.Conclusion MR can evaluate fetal spinal cord and vertebral lesions.It can provide a reliable imaging basis for the diagnosis of fetal spinal cord disease.
