摘  要:目的 探讨全视野器官剂量调制(ODM)技术在女性化疗患者颈、胸、腹、盆CT增强中对辐射剂量及图像质量的影响.方法 将行颈、胸、腹、盆CT增强检查的80例女性化疗患者分为A、B2组,每组40例.A组应用全视野ODM技术,B组应用常规扫描模式.扫描结束后计算和记录所有图像的信噪比(SNR)、对比噪声比(CNR)、主观评分值、甲状腺、乳腺、卵巢层面前、左、后、右方向的管电流值平均值及辐射剂量平均值,对上述数据采用独立样本t 检验进行统计学分析.结果 2组患者动脉期和静脉期甲状腺、乳腺、卵巢层面图像的SNR、CNR及主观评分值无统计学差异(P>0.05).A组患者在甲状腺、乳腺、卵巢层面前、左、后、右方向动脉期及静脉期的平均管电流均低于B组(t=-4.63,t=-4.32,P<0.05);A组患者容积CT剂量指数(CTDIvol)及剂量长度乘积(DLP)平均值较B组分别降低22%(t=-4.77)和24%(t=-4.94),颈部、胸部及腹盆部有效剂量(ED)平均值较B组分别降低19.3%(t=-4.03)、21.4%(t=-3.34)和26.4%(t=-5.68)(P<0.05).结论 在女性化疗患者颈、胸、腹、盆CT增强检查中应用全视野ODM技术可降低患者的辐射剂量,尤其是对电离辐射敏感器官的辐射剂量,而图像质量满足临床诊断标准.Objectivee To investigate the effect of full scan field of view organ dose modulation(ODM)on radiation dose and image quality in cervical-thoracic-abdominal-pelvic enhanced CT in female patients with chemotherapy.Methods A total of 80 female patients with chemotherapy underwent cervical-thoracic-abdominal-pelvic enhanced CT examination were divided into two groups,including group A(full-field ODM group,n=40)and group B(conventional group,n=40).The signal-to-noise ratio(SNR),contrast-to-noise ratio(CNR),subjective score value,the mean value of tube current and mean value of radiation dose of anterior,left,posterior,and right directions of the thyroid,breast and ovary layers of all images were recorded and measured,respectively.All the parameters were further analyzed via independent sample t test between the two groups.Results There were no statistically significant differences in SNR,CNR and subjective scores values between group A and group B in arterial and venous phases of the thyroid,breast and ovary layers(P>0.05).The mean tube current of anterior,left,posterior and right directions of the thyroid,breast and ovary layers in both arterial and venous phases of group A were significantly lower than those of group B(t=-4.63,t=-4.32,P<0.05).The mean volume CT dose index(CTDIvol)and the mean dose length product(DLP)of group A were significantly reduced by 22%(t=-4.77)and 24%(t=-4.94),respectively,comparedto group B.The mean effective dose(ED)of the neck,chest and abdominal-pelvic of group A were significantly decreased by 19.3%(1=-4.03),21.4%(1=-3.34)and 26.4%(1=-5.68),rspectively(P<0.05),compared 1o group B Conclusion The 0DM appears to be effective to reduce the radiation dose of female patients with chemotherapy underwent cervical-thoracic abdominal pelvic enhanced CT examination,especially the radiation dose of radiation-sensitive organs,with maintaining the image quality in clinical practice.
