摘　　要：目的探讨磁共振高分辨率血管壁成像(HRMR-VWI)在大脑中动脉粥样硬化斑块性质评估中的作用。方法选取2019年3月至2021年10月于本院确诊的大脑中动脉狭窄患者84例。根据患者的临床表现、美国国立卫生研究院卒中量表(NIHSS)评分以及磁共振图像表现将患者分为有症状组48例和无症状组36例。同时收集所有患者的临床特征资料及斑块的特点。比较2组患者在临床资料之间的差别及2组患者中斑块在磁共振特征的不同。结果2组患者间主要临床指标差异无统计学意义(P>0.05)。2组患者大脑中动脉粥样硬化斑块在血管管腔狭窄程度及血管重构指数之间比较差异无统计学意义(P>0.05);症状组血管偏心程度及斑块强化程度较无症状组高。结论HRMR-VWI在对大脑中动脉粥样硬化狭窄斑块的定量评估中有重要的临床价值。Objective To investigate the role of magnetic resonance high-resolution vessel wall imaging(HRMR-VWI)in assessing the properties of middle cerebral atherosclerotic plaques.Methods A total of 84 patients with middle cerebral artery stenosis diagnosed in our hospital from March 2019 to October 2021 were selected.According to the clinical manifestations,National Institutes of Health stroke scale(NHISS)scores and magnetic resonance image manifestations of the patients,the patients were divided into symptomatic group(n=48)and asymptomatic group(n=36).At the same time,the characteristics of clinical data and plaque of all patients were collected.The differences in clinical data and in the magnetic resonance characteristics of plaques between the two groups were compared.Results There were no significant differences in the main clinical indicators between the two groups(P>0.05).There were no significant differences in the degree of vascular stenosis and vascular remodelling index of middle cerebral atherosclerotic plaque between the two groups(P>0.05).The degree of eccentricity of blood vessels and the degree of plaque enhancement in the symptomatic group were higher than those in the asymptomatic group(P<0.05).Conclusion HRMR-VWI has important clinical value in the quantitative assessment of atherosclerotic stenotic plaques in the middle cerebral arteries.
