哌替啶联合阿托品肌肉注射用于急诊结石性肾输尿管绞痛的疗效评价

摘　　要：目的哌替啶联合阿托品肌肉注射用于急诊结石性肾输尿管绞痛的疗效评价。方法选择2020年1月-2021年6月浙江省温州市中医院结石性肾输尿管绞痛患者80例,按照随机数字表法分为联合阿托品组和对照组,每组40例。对照组采用哌替啶联合山莨菪碱肌肉注射,其中哌替啶100mg,山莨菪碱10mg;联合阿托品组采用哌替啶联合阿托品肌肉注射,其中哌替啶100mg,阿托品0.5mg。比较两组临床镇痛效果及安全性,并观察首次止痛后的疼痛再发率及两周内肾积水减轻情况。结果联合阿托品组镇痛的总有效率高于对照组;两组首次给药镇痛后0.5小时、6小时及3天内的疼痛再发率比较,联合阿托品组较对照组更低;治疗两周后肾积水减轻率联合阿托品组较对照组更高;以上差异均有统计学意义(P<0.05);两组嗜睡、出汗、头晕、恶心、低血压等不良反应发生率比较差异无统计学意义(P>0.05)。结论急诊结石性肾输尿管绞痛患者接受哌替啶联合阿托品肌肉注射治疗效果显著,疼痛再发率显著降低,两周内肾积水减轻率显著提高,且不良反应未增加,具有临床应用价值。Objective To evaluate the therapeutic effect of piperidine and Atropine internal muscle injection in emergency stone renal ureteric colic.Methods 80 patients from January 2020 to June 2021 were divided into treatment group and control group according to the random number table method,with 40 patients in each group.The control group was treated with piperidine combined with the intramuscular mountain base injection,and the observation group was treated with the piperidine combined with Atropine internal muscle injection.Comparing the clinical analgesic effect and safety between the two groups,and observe the pain recurrence rate after the first pain relief and the reduction of hydronephrosis within two weeks.Results The total response rate of analgesia in the observation group was higher than that of the control group(P>0.05);the rate of pain recurrence in 0.5 hours,6 hours and 3 days after the first analgesic administration was lower than that in the control group(P>0.05).The reduction rate of hydronephrosis was higher than the control group(P>0.05);the incidence of drowsiness,sweating,dizziness,dizziness,nausea and hypotension was not significantly different(P>0.05).Conclusion Patients with emergency stone renal ureteric colic receiving piperidine and atropine intramuscular injection had significant results,significantly lower rate of pain recurrence,significantly reduced rate of hydronephrosis within two weeks,and no improved adverse effects,which has clinical application value.
