摘 要:目的分析早孕期胎儿结构异常的类型及妊娠结局。方法选择在浙江省湖州市妇幼保健院2019年10月-2021年11月接受早孕期胎儿结构异常筛查的4350例,所有孕妇孕11-13^(+6)周接受超声检查并随访至妊娠结束,以引产病理检查、出生后超声检查结果为依据,记录胎儿结构异常情况及分娩结局。结果病理检查、新生儿超声检查结果显示,4350例孕妇中97例(2.23%)在早孕期确诊为胎儿结构异常,其中颈部异常27例(27.84%),中枢神经系统异常22例(22.68%),心血管系统异常16例(16.49%),泌尿系统异常9例(9.28%),骨骼四肢异常8例(8.25%),消化系统异常7例(7.22%),其他类型异常5例(5.15%),颜面异常3例(3.09%);97例中8例经孕中期超声复查无异常,选择继续妊娠,出生后2个月随访未见异常,其余89例因仍有异常而选择终止妊娠。结论早孕期胎儿结构异常发生率为2.23%,结构异常最常见的部位依次是颈部、中枢神经系统和心血管系统,但仍需在孕中晚期进行超声检查,部分胎儿仍有正常分娩的可能。Objective To analyze the types of early pregnancy fetal structural abnormalities and pregnancy outcomes.Method Selected in October 2019-November 2021 in the early pregnancy fetal structural abnormalities screening of 4350 cases of pregnant women as the research object,all pregnant women 11 to 13^(+6)weeks pregnant in Huzhou city,Zhejiang province maternal and child health hospital ultrasound and follow-up to the end of pregnancy,based on pathological examination,postnatal ultrasound results,record fetal structural abnormalities and birth outcomes.Results According to the pathological examination and the neonatal ultrasound examination results,In 97 of 4350 pregnant women(2.23%),they confirmed fetal structural abnormalities in early pregnancy,Among them,27 cases(27.84%)had neck abnormalities,22(22.68%),abnormal cardiovascular system,16(16.49%),Nine cases of urinary abnormalities(9.28%),Eight cases of abnormal skeletal limbs(8.25%),Seven cases of digestive system abnormalities(7.22%),5(5.15%),3 cases of abnormal face(3.09%);In 8 out of 97 cases,ultrasound review was not abnormal in the middle of pregnancy,Choose to continue the pregnancy,No abnormalities were observed at the follow-up 2 months after birth,The remaining 89 cases chose to terminate the pregnancy as they was still abnormal.Conclusion The incidence of fetal structural abnormalities in early pregnancy is 2.23%,and the most prone to structural abnormalities are neck abnormalities,central nervous system and cardiovascular system,but relevant ultrasound examination is still required in the second and third trimester of pregnancy,and some fetuses still may have normal delivery.
