摘　　要：目的 评估甘肃省贫困听障儿童救治项目的实施效果,对项目的成本效益进行卫生经济学评价,为项目的改进提供依据。方法 针对2011年5月—2020年5月参与项目的患儿,通过现场调查收集资料,并且结合甘肃省卫生健康委员会项目数据、甘肃省统计年鉴数据,分析甘肃省听力救治项目的成本和效益,并对项目复筛率进行敏感性分析。结果 甘肃省项目开展期间平均听力筛查率为89.47%,平均听力复筛率为23.04%;最终确定听力救助患儿985例。项目10年总成本21 587.17万元,人均成本20.38万元;总效益为54 885.86万元,人均效益55.72万元。项目开展期间每年的成本效益比在1∶1.90~1∶3.72,10年的总成本效益比为1∶2.54。结论 政府将人工耳蜗与听力筛查相结合进行投资是可行的,目前甘肃省听力救助项目还存在提升空间,建议对项目管理以及资金分配进行进一步的优化。Objective To evaluate the effectiveness of the implementation of the Gansu Provincial Poor Hearing Impaired Children’s Treatment Program, and to evaluate the cost-effectiveness of the program in terms of health economics to provide a basis for program improvement.Methods For the children who participated in the project from May 2011 to May 2020, we collected data through on-site surveys and analyzed the costs and benefits of the hearing treatment project in Gansu Province by combining data from the Health Commission of Gansu Province and the Gansu Provincial Statistical Yearbook, and conducted sensitivity analysis on the re-screening rate of the project.Results The average hearing screening rate was 89.47% and the average hearing re-screening rate was 23.04% during the project period in Gansu Province;985 children were identified for hearing assistance.The total cost of the project over 10 years was 215 871 700 yuan, with a per capita cost of 203 800 yuan;the total benefit was 548 858 600 yuan, with a per capita benefit of 557 200 yuan.The cost-benefit ratio for each year of the project ranged from 1∶1.90 to 1∶3.72, and the total cost-benefit ratio for 10 years was 1∶2.54.Conclusion It is feasible for the government to invest in cochlear implants in combination with hearing screening, and there is still room for improvement in the hearing aid program in Gansu Province.
