宣肺止咳汤辅助治疗风邪犯肺证咳嗽变异性哮喘的临床疗效观察

摘　　要：目的:宣肺止咳汤辅助治疗风邪犯肺证咳嗽变异性哮喘(Cough variant asthma,CVA)的临床疗效。方法:选取我院2019年7月至2020年10月收治的82例风邪犯肺证CVA患者,根据治疗方案不同,分为西药组(41例)和中西药联合组(41例)。西药组在对症治疗的基础上给予孟鲁司特钠+布地奈德治疗,中西药联合组在西药组的基础上给予宣肺止咳汤进行辅助治疗。治疗2 w后,评估两组临床疗效;采用咳嗽症状评分评估患者的咳嗽症状,采用全自动血细胞分析仪检测血液嗜酸性粒细胞计数,采用酶联免疫吸附法检测患者血清中白细胞介素1β(Interleukin1β,IL-1β),莱切斯特咳嗽评分(Chinese Leicester Questionnaire,LCQ)评估患者的生活质量。结果:治疗后,中西药联合组的总有效率(92.68%)高于西药组(70.73%)(P<0.05);与治疗前比较,两组咳嗽症状评分、血液嗜酸性粒细胞计数、IL-1β均降低,LCQ评分升高(P<0.05),且中西药联合组改善更为显著(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:宣肺止咳汤辅助治疗风邪犯肺证CVA临床疗效确切,可有效改善患者的咳嗽症状,减轻气道炎症,降低气道高反应性,提升患者的生活质量。Objective:To observe the clinical effect of Xuanfei Zhike decoction in treatment of cough variant asthma(CVA)of wind evil invading lung syndrome.Methods:82 patients with CVA with the syndrome of wind evil invading lung admitted from July 2019 to October 2020 in our hospital were divided into western medicine group(41 cases)and combination group(41 cases)according to different treatment plans.On the basis of symptomatic treatment,the western medicine group was treated with montelukast sodium+budesonide,while the combination group was treated with Xuanfei Zhike decoction on the basis of the western medicine group.After 2 weeks of treatment,the clinical effects of the two groups were evaluated;the cough symptom score was used to evaluate the patients cough symptoms,the eosinophil count in blood was detected by automatic blood cell analyzer,and the serum level of interleukin-1β(IIL-1β).was detected by enzyme-linked immunosorbent assay The Chinese Leicester Questionnaire(LCQ)was used to evaluate the quality of life of patients.Results:After treatment,the total effective rate of the combination group(92.68%)was higher than that of the western medicine group(70.73%)(P<0.05);Compared with before treatment,cough symptom score,blood eosinophil count,IL-1βwere decreased,the LCQ score was increased in both groups,especially in the combination group(P<0.05).Adverse reactions had no significant difference between the two groups(P>0.05).Conclusion:Xuanfei Zhike decoction has a definite clinical effect in the auxiliary treatment of CVA with the syndrome of wind evil invading lung.It can effectively improve the cough symptoms of patients,reduce airway inflammation,reduce airway hyperresponsiveness,and improve the quality of life of patients.
