彩色多普勒超声在诊断左肾静脉压迫综合征中的临床意义表达

摘　　要：目的:探讨彩色多普勒超声对左肾静脉压迫综合征(Nutcracker syndrome,NCS)的诊断价值。方法:选取90例我院2019年4月至2022年4月NCS患者作为研究组,另选取90例同期健康体检者作为参照组,测量两组不同体位时左肾静脉(Left renal vein,LRV)超声参数及不同部位肾动脉内径、比例。结果:与参照组比较,研究组仰卧位、站立位时狭窄段血管前后径、近肾段血管峰值速度较低,狭窄段血流峰值速度、近肾段血管内径较高(P<0.05);仰卧位、站立位时b/a、压力阶差均较高(P<0.05);脊柱前伸静脉内径、肠系膜上动脉(Superior mesenteric artery,SMA)与腹主动脉(Abdominal aorta,AO)夹角、≥30°比例较低,脊柱左侧肾静脉内径、扩张部与狭窄部内径比、SMA、AO间夹角≤15°、16°~20°比例较高(P<0.05)。结论:彩色多普勒超声对于NCS具有较高诊断价值,临床可通过其进行早期筛查诊断。Objective:To investigate the diagnostic value of color Doppler ultrasound in left renal vein compression syndrome(NCS).Methods:A total of 90 patients with NCS in our hospital from April 2019 to April 2022 were selected as the study group,and 90 healthy people in the same period were selected as the reference group.The ultrasonic parameters of left renal vein(LRV)and the diameter and proportion of renal arteries in different parts of the two groups at different positions were measured.Results:Compared with the control group,in the study group,the anterior and posterior diameters of the stenosis segment and the peak velocity of the proximal renal segment were lower,while the peak velocity of the stenosis segment and the inner diameter of the proximal renal segment were higher in the supine and standing positions(P<0.05);the study group had higher b/a and pressure gradient in supine position and standing position(P<0.05);the study group had a lower proportion of the inside diameter of the spinal anterior extension vein,the angle between the superior mesenteric artery(SMA)and the abdominal aorta(AO)≥30°,and a higher proportion of the inside diameter of the renal vein on the left side of the spine,the ratio of the inside diameter of the expansion to the stenosis,the angle between SMA and AO,≤15°,16°-20°(P<0.05).Conclusion:Color Doppler ultrasound has high diagnostic value for NCS,which can be used for early screening and diagnosis in clinic.
