经阴道超声联合孕酮、β-绒毛膜促性腺激素诊断孕早期先兆流产的价值分析

摘　　要：目的 观察经阴道超声联合孕酮(P)、β-绒毛膜促性腺激素(β-HCG)对孕早期先兆流产的诊断价值。方法 选取30例孕早期先兆流产患者为研究组,30例孕早期产检产妇为对照组。比较两组超声指标[孕囊绒毛间隙血流的收缩期峰值流速(Vps)、收缩期上升支加速度时间(ACT)及阻力指数(RI)]、血清指标(P、β-HCG),评价妊娠黄体回声。结果 研究组孕囊绒毛间隙血流的Vps、P、β-HCG均低于对照组,ACT长于对照组,RI大于对照组,差异有统计学意义(P<0.05)。研究组超声妊娠黄体回声中后壁双环型(63.33%)低于对照组(86.67%),不均质低回声型(36.67%)高于对照组(13.33%),差异有统计学意义(P<0.05)。结论 孕早期先兆流产患者经阴道超声联合P、β-HCG检测,可以弥补单一检测的缺点,具有较高的临床诊断价值。Objective To observe the diagnostic value of transvaginal ultrasound combined with progesterone(P)and p-chorionic gonadotropin(p-HCG)in the occurrence of threatened abortion in early pregnancy.Methods 30 cases of threatened abortion in early pregnancy were study group and 30 cases of pregnant women were control group.The ultrasound indexes[peak systolic velocity(Vps),acceleration time(ACT)and resistance index(RI)of blood flow in the intervillus space of the gestational sac]and serum indexes(P,P-HCG)were compared between the two groups to evaluate the corpus luteum echo of pregnancy.Results The Vps,P and p-HCG of intervillus blood flow in the gestational sac in the study group were lower than those in the control group,the ACT was longer than that in the control group,and the RI was larger than that in the control group,the difference was statistically significant(P<0.05).The middle and rear wall double-ring echo(63.33%)in the study group was lower than that in the control group(86.67%),and the heterogeneous hypoecho type(36.67%)was higher than that in the control group(13.33%),the difference was statistically significant(P<0.05).Conclusion In patients with threatened abortion in early pregnancy,transvaginal ultrasound combined with P and p-HCG detection can make up for the shortcomings of single detection,and has high clinical diagnostic value.
