多层螺旋CT在急性阑尾炎诊断中的应用价值探讨

摘　　要：目的研究多层螺旋CT对急性阑尾炎的诊断价值。方法129例疑似急性阑尾炎患者,均实施多层螺旋CT诊断。比较病理诊断及多层螺旋CT诊断对急性阑尾炎的阳性检出情况;将手术病理诊断结果作为金标准,分析多层螺旋CT对急性阑尾炎的诊断价值、可信度积分。结果①129例患者经病理诊断检出阳性120例,多层螺旋CT检出阳性117例。两种方法对急性阑尾炎的阳性检出率比较差异无统计学意义(P>0.05)。②多层螺旋CT诊断急性阑尾炎的阳性预测值、阴性预测值、敏感度、特异度、准确度、约登指数、误诊率、漏诊率分别为98.29%(115/117)、58.33%(7/12)、95.83%(115/120)、77.78%(7/9)、94.57%(122/129)、0.7361、22.22%(2/9)、4.17%(5/120)。③多层螺旋CT对急性阑尾炎不同部位诊断的可信度积分为(4.19±1.07)分。结论急性阑尾炎具有典型的多层螺旋CT征象,利用多层螺旋CT可以全方面了解阑尾炎及其周围组织的状况,具有较高的检出率和敏感度,可以为临床的诊断和治疗提供科学的依据。Objective To investigate the value of multi-slice spiral CT in diagnosis of acute appendicitis.Methods 129 patients with suspected acute appendicitis were diagnosed by multi-slice spiral CT.The positive detection of acute appendicitis by pathological diagnosis and multi-slice spiral CT diagnosis were compared;the results of surgical pathology were used as the gold standard to analyze the diagnostic value and credibility score of multi-slice spiral CT for acute appendicitis.Results(i)Of the 129 patients,120 cases were found positive by pathological diagnosis,and 117 cases were found positive by multi-slice spiral CT.There was no statistically significant difference in the positive detection rate of acute appendicitis between the two methods(P>0.05).(ii)The positive predictive value,negative predictive value,sensitivity,specificity,accuracy,Yoeden index,misdiagnosis rate and missed diagnosis rate of multi-slice spiral CT in the diagnosis of acute appendicitis were 98.29%(115/117),58.33%(7/12),95.83%(115/120),77.78%(7/9),94.57%(122/129),0.7361,22.22%(2/9),4.17%(5/120).(iii)The reliability scores of multi-slice spiral CT in the diagnosis of different parts of acute appendicitis were(4.19±1.07)points.Conclusion Acute appendicitis has typical signs of multi-slice spiral CT.The use of multi-slice spiral CT can provide a comprehensive understanding of the condition of appendicitis and its surrounding tissues,with a high detection rate and sensitivity,which can provide a scientific basis for clinical diagnosis and treatment.
