产前筛检中甲状腺激素联合甲状腺过氧化物酶抗体的临床意义

摘　　要：目的探究产前甲状腺激素联合甲状腺过氧化物酶抗体(TPOAb)筛检的价值。方法96例孕妇,根据促甲状腺激素(TSH)水平与TPOAb水平分为试验组(异常者,50例)与对照组(正常者,46例)。比较两组甲状腺激素指标[血清游离三碘甲状腺原氨酸(FT_(3))、血清游离甲状腺素(FT_(4))、TSH]、TPOAb水平、妊娠并发症发生情况。结果试验组FT_(3)水平(1.48±0.24)pmol/L、FT_(4)水平(7.21±1.11)pmol/L低于对照组的(4.87±0.36)、(15.66±1.93)pmol/L,TSH水平(5.92±1.42)μIU/ml高于对照组的(2.33±0.28)μIU/ml,差异有统计学意义(P<0.05)。试验组妊娠并发症发生率18.00%高于对照组的4.35%,差异有统计学意义(P<0.05)。试验组TPOAb水平(77.02±43.13)IU/ml高于对照组的(10.34±1.25)IU/ml,差异有统计学意义(P<0.05)。结论将甲状腺激素与TPOAb联用于产前筛检中的效果确切,利于妊娠结局的改善,需给予高度重视。Objective To investigate the value of thyroid hormone combined with thyroid peroxidase antibodies(TPOAb)in prenatal screening.Methods A total of 96 pregnant women were divided into an experimental group(abnormal,50 cases)and a control group(normal,46 cases)according to thyroid stimulating hormone(TSH)and TPOAb levels.The thyroid hormone indices[serum free triiodothyronine(FT_(3)),serum free thyroxine(FT_(4)),TSH],TPOAb levels,and occurrence of pregnancy complications were compared between the two groups.Results The FT_(3)(1.48±0.24)pmol/L and FT_(4)(7.21±1.11)pmol/L in the experimental group were lower than(4.87±0.36)and(15.66±1.93)pmol/L in the control group;the TSH(5.92±1.42)μIU/ml in the experimental group was higher than(2.33±0.28)μIU/ml in the control group;the differences were statistically significant(P<0.05).The incidence of pregnancy complications in the experimental group was 18.00%,which was higher than 4.35%in the control group,and the difference was statistically significant(P<0.05).The TPOAb(77.02±43.13)IU/ml in the experimental group was higher than(10.34±1.25)IU/ml in the control group,and the difference was statistically significant(P<0.05).Conclusion Tthyroid hormone in combination with TPOAb in prenatal screening has a definite effect and is conducive to the improvement of pregnancy outcome,which needs to be paid more attention.
