摘 要:目的探究糖尿病与听力损失的关联。方法采用横断面研究,共纳入2014~2017年参加健康体检的开滦集团在职及离退休男性职工20824人。听力损失的诊断参照世界卫生组织(WHO)听力损失诊断标准,根据是否糖尿病将观察对象分为糖尿病组和非糖尿病组。采用Logistic回归模型分析糖尿病与听力损失的关联,并根据噪声暴露与否进行亚组分析。结果校正潜在的混杂因素后,糖尿病与言语频率听力损失及高频听力损失呈正向关联,与低频听力损失无关;其OR值(95%CI)分别为:1.22(1.05~1.42)、1.17(1.03~1.33)、1.14(0.98~1.33)。亚组分析中,在校正相同因素后,在非噪声组中所得结果与以上一致,其OR值(95%CI)分别为:1.25(1.05~1.48)、1.20(1.04~1.39)、1.17(0.99~1.39)。在噪声组中,糖尿病与各类别听力损失均无关。结论男性糖尿病与听力损失呈正向关联,且主要影响高频听力。OBJECTIVE To explore the association between diabetes mellitus(DM)and hearing loss(HL).METHODS Using cross-sectional study,20824 male employees of Kailuan Group who participated in physical examination from 2014 to 2017 were included in the study.HL was diagnosed according to the WHO diagnostic criteria for hearing loss,and the subjects were divided into DM group and non DM group according to whether they were DM or not.Logistic regression model was used to analyze the association between DM and HL,and subgroup analysis was conducted according to whether the noise was exposed or not.RESULTS After correcting for potential confounders,DM was positively correlated with language frequency HL and high frequency HL,but not with low frequency HL;The OR values(95%CI)were 1.22(1.05-1.42),1.17(1.03-1.33)and 1.14(0.98-1.33),respectively.By subgroup analysis,after adjusting for the same factors,the results obtained in the non-noise group were consistent with the above,and their OR values(95%CI)were 1.25(1.05-1.48),1.20(1.04-1.39)and 1.17(0.99-1.39),respectively.In the noise group,DM is independent of each category HL.CONCLUSION In males,DM is positively correlated with HL and mainly affects high-frequency hearing.
