摘 要:目的研究血浆tRF-19-DRXSE5I2在4~6岁阻塞性睡眠呼吸暂停低通气综合征(OSAHS)患儿中的差异表达及其潜在的诊断筛查价值。方法研究纳入2020年11月~2021年12月在宁波市妇女儿童医院确诊的45例OSAHS患儿及同期同年龄段33名健康对照儿童,记录两组儿童临床资料。前期两组各留取5份血浆样本进行高通量测序,初步筛选出差异表达的转运RNA衍生小RNA(tRNA-derived small RNAs,tsRNAs),随后对所有样本进行实时荧光定量逆转录-聚合酶链式反应(qRT-PCR)验证,分析其诊断筛查价值及与临床病理资料的关系。结果基于高通量测序结果,选择tRF-19-DRXSE5I2作为研究对象。qRT-PCR验证发现血浆tRF-19-DRXSE5I2在OSAHS患儿中表达明显下调(P<0.0001),与高通量测序结果一致。受试者工作特征(ROC)曲线显示tRF-19-DRXSE5I2诊断OSAHS的ROC曲线下面积(area under curve,AUC)为0.8694,灵敏度为82.22%,特异度为81.82%(P<0.0001)。相关性分析显示差异表达的tRF-19-DRXSE5I2与免疫球蛋白E(IgE)具有相关性。结论血浆tRF-19-DRXSE5I2在OSAHS患儿中低表达,且和IgE相关,可能成为儿童OSAHS潜在的诊断标志物。OBJECTIVE To investigate the expression and potential diagnostic value of plasma tRF-19-DRXSE5I2 in 4-6 years old children with obstructive sleep apnea-hypopnea syndrome(OSAHS).METHODS A total of 45 OSAHS children and 33 healthy control children in Ningbo Women and Children’s Hospital from November 2020 to December 2021 were enrolled.The clinical data of the two groups were recorded.At first,five plasma samples were collected from each group for high-throughput sequencing.Secondly,differentially expressed tRNA-derived small RNAs(tsRNAs)were screened out and were verified by real-time quantitative reverse transcription-polymerase chain reaction(qRT-PCR)on all samples.Then,the diagnostic value of tsRNAs and the relationship with clinical data were analyzed.RESULTS Based on the results of highthroughput sequencing,tRF-19-DRXSE5I2 was selected as the study object.qRT-PCR showed that the expression of plasma tRF-19-DRXSE5I2 was significantly down-regulated in children with OSAHS(P<0.0001),which was consistent with the results of high-throughput sequencing.Receiver operating characteristic curve(ROC)showed that the area under curve(AUC)of tRF-19-DRXSE5I2 was 0.8694 with 82.22%sensitivity and 81.82%specificity(P<0.0001).Correlation analysis showed that tRF-19-DRXSE5I2 was correlated with immunoglobulin E(IgE).CONCLUSION The expression level of plasma tRF-19-DRXSE5I2 was decreased in OSAHS children and was related to IgE,which may become a potential biomarker for the diagnosis of pediatric OSAHS.
