摘　　要：目的探讨原因不明不育男性患者8-羟基脱氧鸟苷(8-OHdG)、瘦素(LEP)、锌α2糖蛋白(ZAG)及封闭抗体的临床诊断价值。方法选取2021年1月至2022年1月我院收治的原因不明不育男性患者120例作为研究组,另选同期生育功能正常的健康男性110例为对照组。分析对比研究组和对照组8-OHdG、LEP、ZAG及封闭抗体的阳性表达数、水平、临床诊断价值以及与卵泡生成激素(FSH)、黄体生成激素(LH)、睾酮(T)水平的相关性。结果与对照组比较,研究组患者8-OHdG、LEP、ZAG及封闭抗体阳性表达数较多(P<0.05);与对照组相比,研究组8-OHdG、ZAG明显高于对照组,而LEP、及封闭抗体水平低于对照组(P<0.05);在原发型中8-OHdG阳性多见,在继发型中LEP、ZAG阳性多见,而在反复流产型中封闭抗体阳性多见;可知在原发型中8-OHdG水平明显高于继发型、反复流产型,在继发型中LEP水平明显高于原发型、反复流产型,而ZAG水平明显低于原发型、反复流产型,而在反复流产型中封闭抗体水平低于原发型、继发型(P<0.05);联合检测的AUC、灵敏度、特异性均高于单独检测,差异有统计学意义(P<0.05);研究组的FSH、LH、T的水平明显高于对照组(P<0.05);显示FSH、LH与8-OHdG、LEP呈正相关(r=0.36,0.29,P均<0.05),与ZAG及封闭抗体呈负相关(r=-0.31,P<0.05)。结论通过对原因不明不育男性患者8-OHdG、LEP、ZAG及封闭抗体的检测,有助于诊断患者的病情,指导临床选择合适的治疗方案。Objective To investigate the clinical diagnostic value of 8-hydroxydeoxyguanosine(8-OHdG),leptin(LEP),Zincα2 glycoprotein(ZAG)and blocking antibody in male patients with unexplained infertility.Methods A total of 120 male patients with unexplained infertility admitted to our hospital from January,2021 to January,2022 were selected as the research group,while 110 healthy men with normal fertility function were selected as the control group during the same period.The positive expressions of 8-OHdG,LEP,ZAG and blocking antibodies in the study group and the control group were analyzed and compared in terms of the number,level,clinical diagnostic value and correlation with follicle-producing hormone(FSH),luteinizing hormone(LH)and testosterone(T)levels.Results Compared with the control group,there were higher levels of positive expressions of 8-OHdG,LEP,ZAG and blocking antibody in the study group(P<0.05).Compared with the control group,levels of 8-OHdG and ZAG in the study group were significantly higher than those in the control group,while levels of LEP and blocking antibody in the study group were lower than those in the control group(P<0.05).The positive rate of 8-OHdG was more common in the primary hairstyle,LEP and ZAG were more common in the secondary hairstyle,and blocking antibody was more common in the recurrent abortion type.The level of 8-OHdG in the original hairstyle was significantly higher than that in the successor hairstyle and recurrent abortion type,and the level of LEP in the successor hairstyle was significantly higher than that in the original hairstyle and recurrent abortion type,while the level of ZAG was significantly lower than that in the original hairstyle and recurrent abortion type,and the level of blocking antibody in the recurrent abortion type was lower than that in the original hairstyle and recurrent abortion type(P<0.05).AUC,sensitivity and specificity of combined detection were higher than those of single detection(P<0.05).Levels of FSH,LH and T in the study group were significa
