摘  要:斑秃是临床上常见的非瘢痕性脱发,严重者可出现全秃、普秃。其发病机制复杂,治疗面临诸多困难,存在传统治疗方式治疗中重度斑秃欠佳、不同斑秃患者因发病机制不同存在疗效差异、治疗效果很好的普秃患者存在斑秃复发的风险等问题,患者精神压力大。随着生物制剂及小分子药物在特应性皮炎、银屑病的广泛应用,斑秃的靶向治疗正成为重要的治疗手段。本文综述斑秃免疫发病机制与靶向治疗进展,探讨治疗新思路。Alopecia areata is a common clinical non-cicatricial alopecia.Alopecia totalis and generalized alopecia may occur in severe cases.The pathogenesis of alopecia areata is complex,and the treatment is faced with many difficulties.The traditional treatment method is not effective for moderate and severe alopecia areata.Therapeutic efficacy varies with patients because of the differences in the pathogenesis.Although patients with generalized alopecia areata usually respond well to the treatment,the risk of recurrence is high.Patients with generalized alopecia areata are often subject to psychological stress.With the wide use of biological agents and small molecular drugs in atopic dermatitis and psoriasis,targeted treatment for alopecia areata is becoming an important treatment method.This article reviews the progress of immunopathogenesis and targeted treatment of alopecia areata,and explores new ideas for the treatment.
