2019-2021年度签约高血压患者社区卫生服务机构就诊情况的帕累托图分析

摘　　要：目的:分析2019—2021年新型冠状病毒感染疫情期间闵行区华漕社区卫生服务中心签约高血压患者的社区就诊疾病构成及就诊情况变化。方法:选取于2019年1月1日—2021年12月31日在华漕社区卫生服务中心就诊的7907例“1+1+1”签约高血压患者作为研究对象,收集患者的就诊相关信息,比较不同年份间的就诊情况差异。结果:2019—2021年签约高血压患者疾病分类顺位及各单病种疾病顺位基本一致,2019—2021年签约高血压患者前5种疾病分类分别为循环系统疾病,内分泌、营养和代谢疾病,消化系统疾病,呼吸系统疾病,肌肉骨骼系统和结缔组织疾病;2019—2021年签约高血压患者前10位单病种疾病分别为高血压、冠状动脉粥样硬化性心脏病、糖尿病、上呼吸道感染、高脂血症、失眠症、脑梗死后遗症、慢性胃炎、便秘和关节炎。由帕累托图可见,2019—2021年签约高血压患者主要就诊疾病为高血压、冠状动脉粥样硬化性心脏病、糖尿病、上呼吸道感染及高脂血症。2019—2021年,在不同医生处就诊、在不同场所就诊及采用不同预约挂号方式就诊的签约高血压患者日均就诊例次数差异均有统计学意义(P<0.05)。结论:社区卫生服务机构应大力开展家庭医生签约服务及诊疗能力、分级诊疗制度、互联网医院等的建设工作,以有效应对高血压患者的就医需求变化。Objective:To analyze the changes in the composition of diseases seen and visits of contracted hypertensive patients at Huacao Community Health Service Center in Minhang District during the COVID-19 from 2019 to 2021.Methods:From January 1,2019 to December 317907 patients with hypertension who signed the“1+1+1”mode and visited Huacao Community Health Service Center were selected as the study subjects,and the relevant information of patients’visits was collected to compare the differences of visits between different years.Results:The disease classification order of contracted hypertensive patients in 2019-2021 and the disease order of each single disease were basically the same,the top 5 disease classes of contracted hypertensive patients seen in 2019-2021 were circulatory system diseases,endocrine,nutritional and metabolic diseases,digestive system diseases,respiratory system diseases,musculoskeletal system and connective tissue diseases;the top 10 single disease categories of contracted hypertensive patients in 2019-2021 were hypertension,coronary atherosclerotic heart disease,diabetes mellitus,upper respiratory tract infection,hyperlipidemia,insomnia,sequelae of cerebral infarction,chronic gastritis,constipation,and arthritis.As seen in the Pareto chart,the main diseases seen by contracted hypertensive patients in 2019-2021 were hypertension,coronary atherosclerotic heart disease,diabetes,upper respiratory tract infections and hyperlipidemia.In 2019-2021,the differences in the average number of daily visits for contracted hypertensive patients whose number of visits to different physicians,visits to different sites and visits using different appointment registration methods were statistically significant(P<0.05).Conclusion:The community health service institutions should vigorously carry out the construction of family doctors’contract signing and diagnosis and treatment capacity,hierarchical diagnosis and treatment system,Internet hospitals,etc.,to effectively respond to the changes in medical needs of h
