老年头颈部梭形细胞横纹肌肉瘤1例

摘　　要：患者女,86岁,右侧面颊肿物半年,逐渐增大。皮肤科情况:右眼外下方见约2.5 cm×2.5 cm大小肿物,淡红色,突出皮面,表面溃疡,无压痛。皮损组织病理示:肿瘤位于真皮内,由大量呈编织状排列的梭形细胞构成,细胞异型,核分裂象多见。免疫组织化学示:p-CK(-),SMA弱阳性,S100(-),Desmin(+),MyoD1(+),Myogenin(-),CD34(-),ERG(-),Ki-67(约80%+)。诊断:梭形细胞横纹肌肉瘤。予手术切除,随访未见复发。A 86-year-old female presented with a lump on her right cheek for six months, which increased gradually. Dermatological examination showed a tumor about 2.5 cm×2.5 cm on the outside and below of the right eye, which was light red and protruded, and with ulceration on the surface but without tenderness. Histopathology showed the tumor was located in the dermis and consisted of a large number of spindle-shaped cells arranged in a woven pattern, with atypical cells. Mitoses were frequent. Immunohistochemistry showed positive staining for Desmin, MyoD1,SMA(weakly positive), Ki-67(approximately 80%),but negative staining for p-CK,S100,Myogenin, CD34,ERG.The diagnosis was spindle cell rhabdomyosarcoma.Surgical excision was performed, and no recurrence was observed during follow-up.
