摘 要:目的探讨直接数字化X线摄影(DR)检查在儿童胸部检查中的应用价值。方法选取2019年8月~2021年8月在我院疑似胸部病变行胸部检查的儿童243例,均行DR检查、常规X线平片检查,以CT检查结果为“金标准”,比较两种方法胸部异常阳性检出率、摄影参数、摄影图像质量评分、摄影胸片质量分级、非甲级片失误因素。结果3名医生对DR检查图像诊断的一致性ICC值为0.912,95%CI为0.836~0.975;对常规X线图像诊断的一致性ICC值为0.905,95%CI为0.814~0.947,3名医生对两种方法检查图像诊断的一致性ICC值均在0.9以上,表明具有良好一致性;与常规X线平扫比较,DR检查诊断灵敏度、准确度较高(92.31%,96.71%),漏诊率较低(7.69%),差异均有统计学意义(P<0.05);与常规X线检查相比,DR检查曝光量、X线辐射剂量较少,检查时间、曝光时间较短,差异均有统计学意义(P<0.05);与常规X线检查比较,DR检查双肺纹理、气管、心影后方、椎体图像质量评分较高,差异均有统计学意义(P<0.05);与常规X线检查比较,DR检查甲级片较多,丙级片、废片较少,差异均有统计学意义(P<0.05);与常规X线检查比较,DR检查因摄影条件、暗室技术引起非甲级片例数较少,差异均有统计学意义(P<0.05)。结论DR检查可有效提高儿童胸部检查疾病检出率,提高图像质量,减少废片率,从而避免多次检查,降低儿童所受X线辐射剂量。Objective To explore the application value of X-ray examination in children's chest examination.Methods 243 children suspected of chest diseases in our hospital from August 2019 to August 2021 were selected for chest examination.All of them were examined by digital radiography(DR)and conventional plain X-ray.The results of computed tomography(CT)were taken as the"gold standard".The positive detection rate,photographic parameters,photographic image quality score,photographic chest film quality grading Non Class A film error factors were compared between the two methods.Results The consistency of the three doctors'diagnosis of DR images was 0.912,95%CI was 0.836~0.975;the consistent ICC value of conventional X-ray image diagnosis was 0.905,95%CI was 0.814~0.94.The consistency of the three doctors'image diagnosis by the two methods was above 0.9,which was good consistency.Compared with the conventional X-ray plain scan,the diagnostic sensitivity of DR was higher(92.31%),the accuracy was higher(96.71%),and the missed diagnosis rate was lower(7.69%),the differences were statistically significant(P<0.05);Compared with routine X-ray examination,DR examination had less exposure and X-ray radiation dose,and shorter examination time and exposure time,the differences were statistically significant(P<0.05);Compared with routine X-ray examination,DR examination had higher image quality scores of bilateral lung markings,trachea,posterior heart and vertebral body,the differences were statistically significant(P<0.05);Compared with routine X-ray examination,DR examination had more A-grade films,less C-grade films and waste films,the differences were statistically significant(P<0.05);Compared with routine X-ray examination,DR examination had fewer cases of non grade A films due to photographic conditions and dark room techniques,the differences were statistically significant(P<0.05).Conclusion DR examination can effectively improve the detection rate of children's chest diseases,improve the image quality,reduce the rate of waste
