摘　　要：目的:系统分析夫精宫腔内人工授精(IUI)女性病人妊娠率的影响因素。方法:检索Web of Science、PubMed、the Cochrane Library、EMbase、中国生物医学文献数据库、中国知网、万方数据库、维普数据库中有关夫精宫腔内人工授精妊娠率影响因素的队列研究或病例对照研究,检索时限从建库至2022年3月20日。由2名研究员完成筛选文献、提取资料及文献质量评价,采用RevMan 5.4软件进行Meta分析。结果:共纳入12篇文献。Meta分析结果显示,年龄≥36岁[OR=1.22,95%CI(1.08,1.38),P=0.002]、原发性不孕[OR=2.18,95%CI(1.55,3.06),P<0.00001]、不孕年限>5年[OR=1.25,95%CI(1.03,1.51),P=0.02]、优势卵泡数≤1个[OR=1.95,95%CI(1.06,3.59),P=0.03]、授精时机[OR=3.37,95%CI(1.70,6.70),P=0.0005]、无促排卵方案[OR=2.48,95%CI(1.47,4.19),P=0.0007]、IUI周期数>2次[OR=1.91,95%CI(1.63,2.23),P<0.00001]及前向活动精子数量<5×106个[OR=1.50,95%CI(1.25,1.80),P<0.0001]是影响夫精宫腔内人工授精女性病人妊娠率的独立因素。结论:现有证据表明,年龄≥36岁、不孕类型、不孕年限>5年、优势卵泡数≤1个、授精时机、促排卵方案、IUI周期数>2次、前向活动精子数量<5×106个是影响夫精宫腔内人工授精女性病人妊娠率的因素。Objective:To systematically analyze the influencing factors of pregnancy rate in female patients with intrauterine insemination with husband's sperm.Methods:Cohort studies or case-control studies on the factors affecting the rate of pregnancy after intrauterine insemination were retrieved from Web of Science,PubMed,the Cochrane Library,EMbase,CBM,CNKI,WanFang Database and VIP.The retrieval time was from the establishment of the database to March 20,2022.Literature screening,data extraction and literature quality evaluation were completed by two researchers.RevMan 5.4 software was used for Meta-analysis.Results:A total of 12 articles were included.The results of Meta-analysis showed that age≥36 years[OR=1.22,95%CI(1.08,1.38),P=0.002],primary infertility[OR=2.18,95%CI(1.55,3.06),P<0.00001],infertility years>5 years[OR=1.25,95%CI(1.03,1.51),P=0.02],number of dominant follicles≤1[OR=1.95,95%CI(1.06,3.59),P=0.03],timing of insemination[OR=3.37,95%CI(1.70,6.70),P=0.0005],no ovulation induction scheme[OR=2.48,95%CI(1.47,4.19),P=0.0007],number of IUI cycles>2[OR=1.91,95%CI(1.63,2.23),P<0.00001]and number of forward motile sperm<5×106[OR=1.50,95%CI(1.25,1.80),P<0.0001]were independent factors affecting the pregnancy rate of female patients with intrauterine artificial insemination.Conclusion:Existing evidence shows that age≥36 years,infertility type,infertility duration>5 years,the number of dominant follicles≤1,insemination timing,ovulation induction scheme,the number of IUI cycles>2,and the number of forward motile sperm<5×106 are the factors affecting the pregnancy rate of female patients with intrauterine insemination.
