摘　　要：目的:探讨自制肝门阻断钳在腹腔镜下肝切除术中的应用价值。方法:回顾性分析2019年3月—2021年1月60例行腹腔镜下肝切除术患者的临床资料,其中30例术中采用自制肝门阻断钳阻断肝门血流(观察组),30例术中采用传统腔镜下阻断钳阻断肝门血流(对照组),比较两组各项临床指标。结果:观察组阻断肝门操作时间为(3.2±2.1)s,对照组为(22.4±2.5)s,差异有统计学意义(P<0.01);观察组去阻断操作时间为(2.6±0.9)s,对照组为(31.0±5.8)s,差异有统计学意义(P<0.01);观察组术后住院时间为(8.2±1.9)d,对照组为(9.5±1.5)d,差异有统计学意义(P<0.05)。两组术中出血量、术后肝功能、术后并发症发生率等方面差异均无统计学意义(P均>0.05)。结论:自制肝门阻断钳在腹腔镜下肝切除术中操作简单,可优化手术流程,阻断效果可靠。Objective:To assess the application value of self-made external hepatic pedicle occlusion forceps in laparoscopic hepatectomy.Methods:The data of 60 patients undergoing laparoscopic liver tumor resection from March 2019 to March 2021 were retrospectively analyzed.Among them,30 patients were treated with self-made external hepatic pedicle occlusion forceps to block hepatic portal blood flow(observation group),and the other 30 patients were treated with traditional endoscopic occlusion forceps to block hepatic portal blood flow(control group).The clinical indexes of the two groups were compared.Results:The operation time of portal occlusion in the observation group was(3.2±2.1)s,while that in the control group was(22.4±2.5)s,and the difference was statistically significant(P<0.01).The postoperative length of stay in observation group was(8.2±1.9)d,and in control group was(9.5±1.5)d,with statistical significance(P<0.05).The operation time of the remove portal occlusion in the observation group was(2.6±0.9)s,while that of the control group was(31.0±5.8)s,the difference was statistically significant(P<0.01).There were no significant differences in intraoperative blood loss,liver function and incidence of other complications between the two groups(all P>0.05).Conclusion:The self-made external hepatic pedicle occlusion forceps is simple to operate in laparoscopic hepatectomy,greatly optimizing the surgical process,with reliable occlusion effect.
