摘  要:围术期机械通气诱发的呼吸机相关肺损伤是机械通气的严重并发症,是导致术后肺功能不全的重要因素,也越来越受到麻醉医生的重视。肺呼吸功能监测能及时准确发现围术期肺损伤,指导肺保护策略的实施。随着各种检测技术和仪器的发展,肺呼吸功能监测除临床常用的血气分析外,还衍生出更有借鉴意义的肺部超声、电阻抗成像技术、食管测压、综合肺指数监测、无创呼吸量监测、无创连续呼吸监测等方法。这些方法各有利弊,本文就肺呼吸功能监测临床应用进展及优缺点进行综述,旨在为临床提供参考。Ventilation-induced lung injury is a serious complication of mechanical ventilation and an important factor leading to postoperative pulmonary insufficiency,which is increasingly valued by anesthesiologists.Pulmonary respiratory function monitoring can accurate detect preoperative lung injury timely and guide the implementation of lung protection strategies.With the development of various testing techniques and instruments,pulmonary respiratory function monitoring has derived more informative methods such as lung ultrasound,electrical impedance tomography,esophageal manometry,integrated pulmonary index,non-invasive respiratory volume monitor,and non-invasive continuous respiratory monitoring,in addition to blood gas analysis commonly used in clinical practice.Each of these methods has its own advantages and disadvantages.This article reviews the clinical application progress,advantages and disadvantages of pulmonary respiratory function monitoring,aiming to provide clinical reference.
