聚丙烯酰胺水凝胶取出后乳房一期重建与二期重建疗效评价的Meta分析

摘　　要：目的通过Meta分析评价聚丙烯酰胺水凝胶(polyacrylamide hydrogel,PAHG)取出后一期重建与二期重建的疗效。方法检索PubMed、Embase、Cochrane Library、Web of Science、中国知网数据库、万方中文数据库、维普数据库、中国生物医学文献数据库,检索时间自1995年1月至2022年3月,关于PAHG取出后一期重建与二期重建的研究,采用EndNote X9、Rev Man 5.4进行文献筛选和Meta分析。结果纳入7篇临床观察性研究。Meta分析结果表明,PAHG取出后一期重建与二期重建患者术后并发症发生率、生命质量和自尊评分比较,差异无统计学意义(P>0.05)。术后BREAST-Q评分中,乳房满意度、心理健康、躯体健康三项评分差异均无统计学意义(P>0.05),仅性健康一项二期重建组得分较高(P<0.05)。结论PAHG取出后乳房一期重建与二期重建均可有效改善填充物取出所致的乳房形态不佳,可根据医师判断及患者意愿选择合适的时机进行手术。Objective To conduct a Meta-analysis comparing the effect of immediate versus delayed reconstruction after polyacrylamide hydrogel(PAHG)removal.Methods Articles regarding immediate and delayed breast reconstruction after PAHG removal,were searched in PubMed,Embase,Cochrane Library,Web of Science,Chinese journals full-text database,WanFang database,VIP database and China biology medicine disc.EndNote X9 and Revman 5.4 were used for study inclusion and Meta-analysis.Results A total of 7 observational studies were included.No significant difference was found between immediate and delayed reconstruction in the incidence of postoperative complications,quality of life and self-esteem score after PAHG removal(P>0.05).As for the BREAST-Q evaluation,there was no significant difference in satisfaction with appearance of breasts,psychosocial wellbeing and physical wellbeing(P>0.05).Significant difference was only found in sexual wellbeing(P<0.05).Conclusion Immediate and delayed breast reconstruction after the removal of PAHG can both effectively improve the poor breast shape caused by the removal of filler,and appropriate timing can be selected according to the doctor’s judgment and the patient’s will.
