摘 要:目的探讨脂肪来源干细胞(adipose-derived stem cells,ADSCs)对大鼠同种异体皮瓣移植的免疫调节作用。方法自2015年9月至2017年5月,解放军总医院(解放军医学院)通过留取整形术后的脂肪组织,经剪切、消化酶消化、过滤、离心后收集单个细胞;利用贴壁培养法获得贴壁细胞,扩增传代培养ADSCs;流式细胞术检测细胞表型,添加分化培养基后检测细胞的多向分化功能;构建大鼠同种异体皮瓣移植模型,按静脉是否输注ADSCs,将大鼠分为实验组与对照组,观察各组大鼠移植皮瓣及腹股沟淋巴结的外观,检测各组大鼠移植术后3 d皮瓣的CD3阳性细胞分布情况以及腹股沟淋巴结的T细胞亚型分化情况,记录皮瓣成活时间。结果经酶消化法提取的ADSCs,符合间充质干细胞标志特征和多向分化功能;与对照组相比,实验组大鼠移植皮瓣的颜色正常、未见明显红肿,腹股沟淋巴结外观趋于正常,移植皮瓣中CD3阳性细胞数量明显减少(P<0.05),淋巴结内Th2/Th1明显增加(P<0.05),皮瓣成活时间明显增加(P<0.05)。结论利用术后脂肪组织成功分离培养出ADSCs,操作方法简单、细胞功能稳定。静脉输注获取的ADSCs可抑制大鼠异体皮片移植后的免疫反应,ADSCs有望在组织器官再造和免疫治疗中具有良好的应用。Objective To explore the immunomodulatory effect of adipose-derived stem cells(ADSCs)on vascularized composite allotransplantation in rats.Methods From September 2015 to May 2017,the adipose tissue obtained from plastic surgery was sheared,digested with digestive enzymes and filtered to obtain single cells.Then,the adherent cells were obtained and subcultured.The phenotype and multi-directional differentiation potential of the expanded ADSCs were detected.The rat vascularized composite allotransplantation model was constructed,and the rats were divided into the experiment group and the control group according to whether ADSCs were infused intravenously or not.The appearance of the transplanted skin flap and the inguinal lymph nodes were observed,the distribution of CD3positive cells in the skin flaps and the T cell subtype differentiation of the inguinal lymph nodes were detected 3 days after transplantation in each group of rats.Results ADSCs extracted by digestive enzymes were in line with the hallmark characteristics of mesenchymal stem cells and multi-directional differentiation function.Compared with the control group,the color of the transplanted skin flap in the experimental group was normal,without obvious redness and swelling,the appearance of the inguinal lymph nodes tended to be normal,the number of CD3 positive cells in the transplanted skin flap was significantly reduced(P<0.05),and Th2/Th1 increased significantly(P<0.05),and the survival time of the flap increased significantly(P<0.05).Conclusion ADSCs are successfully isolated and cultured from the adipose tissue obtained from plastic surgery.This method is simple in operation and stable in cell function.Intravenous infusion of ADSCS can inhibit the immune response after allogeneic skin grafting in the rat.It is expected to have a promising application in tissue and organ construction and immunotherapy.
