超声诊断在三阴性乳腺癌诊断中的临床意义

摘　　要：目的:分析三阴性乳腺癌(TNBC)与非TNBC的超声图像差异,以探讨TNBC的超声诊断价值。方法:回顾性分析286例浸润性乳腺癌女性患者的超声影像和一般临床资料。比较2组一般资料及声像图特征分布情况,采用多元Logistic回归筛选TNBC的独立超声声像图特征。结果:TNBC 46例(16.1%),非TNBC 240例(83.9%);单因素分析结果显示肿块大小、形状、边缘、后方回声及组织学分级均有统计学差异(P<0.05),肿块最大径>2 cm、形态规则、边缘光整及后方回声增强更倾向于TNBC(33/46,20/46,24/46,17/46),而后方回声衰减更可能是非TNBC的特征(97/240),TNBC组织学分级更高(22/46);多因素Logistic回归分析显示,肿块最大径>2 cm(OR=3.457)、形态规则(OR=0.205)、边缘光整(OR=0.374)是鉴别TNBC的独立影响因素(均P<0.05)。结论:可通过超声检查分析乳腺病灶的形态学特征,为TNBC的检出提供重要参考信息。Objective:To analyze the difference of ultrasound images between triple-negative breast cancer(TNBC)and non-TNBC,so as to explore the value of ultrasound diagnosis of TNBC.Methods:The ultrasound images and general clinical data of 286 female patients with invasive breast cancer were retrospectively analyzed.The t test andχ2 test were used to analyze the differences in the distribution of general data and sonographic features between the two groups,and multivariate Logistic regression was used to screen the independent sonographic features of TNBC.Results:There were 46 cases of TNBC(16.1%),and 240 cases of non-TNBC(83.9%);there were statistically significant differences in the size,shape,edge,rear echo and histological grade of the tumor in single factor analysis(P<0.05),and tumors with a maximum diameter>2 cm,regular shape,smooth edge,and enhanced rear echo were more likely to be TNBC(33/46,20/46,24/46,17/46),while posterior echo attenuation was more likely to be a feature of non-TNBC(97/240).The histological grade of TNBC was higher(22/46).Multivariate Logistic regression analysis showed that the largest diameter of the tumor>2 cm,regular morphology,and smooth margins were independent influencing factors for the identification of TNBC(all P<0.05).Conclusion:The morphological characteristics of breast lesions can be analyzed by ultrasonography,which can provide important reference information for the detection of TNBC.
