摘　　要：冠心病是我国居民死亡的主要病因,经皮冠状动脉介入治疗手术(PCI)是快速有效解除心肌缺血状态,改善心肌循环的首选方法,有助于降低患者的病死率。此外,在PCI术中需要采用抗凝药物治疗,其是当下PCI手术治疗中不可或缺的重要部分,可直接影响手术的预后。PCI是冠心病和血管闭塞的一种常用支架疗法,其特点是疗程短、创伤小、疗效显著。随着近年来医疗技术的进步,新的抗凝药物与抗凝方案不断涌现。传统的抗凝药物普通肝素与新的药物比伐卢定之间孰优孰劣的讨论持续不断,二者均作为一线药物应用于临床,到目前为止,对何种药物作为首选仍无定论。本文通过对2种药物作用原理的异同和几个有代表性的大型研究(REPLACE-2、ISAR-REACT-3、ACUITY、HORIZONS-AMI、BRIGHT)进行总结归纳,为临床PCI抗凝治疗提供参考。Coronary heart disease is the main cause of death in our count percutaneous coronary intervention(PCI)is the first choice to relieve myocardial ischemia and improve myocardial circulation,which is helpful to reduce the mortality of patients.In addition,anticoagulant therapy is necessary in PCI,which is an indispensable part of the current PCI procedure,and it can directly affect the prognosis of the operation.It is a common stent therapy for coronary heart disease and vascular occlusion.It is characterized by short course of treatment,less trauma and remarkable effect.Along with the progress of medical technology in recent years,new anticoagulant drugs and anticoagulant schemes emerge constantly,and the discussion between the traditional anticoagulant drug unfractionated heparin and the new drug bivalirudin is continuous,both of them are used as first-line drugs in clinic,so far,there is no conclusion about which drug is the first choice.In this review,we summarized the principle of two-week anticoagulation therapy and some representative large-scale studies(REPLACE-2,ISAR-REACT-3,ACUITY,HORIZONS-AMI,BRIGHT).
